缺铁性贫血继发血小板增多患者血清EPO、TPO水平检测及临床意义

摘　　要：目的:检测缺铁性贫血(IDA)继发血小板增多的患者血清EPO、TPO水平,探讨IDA继发血小板增多与EPO、TPO关系及其临床意义。方法:对122例确诊为IDA患者(其中44例伴血小板增多及78例血小板数正常)进行研究,分析其临床特征;40例健康体检患者为对照组,ELISA法检测患者及对照组血清EPO、TPO水平,并进行统计分析;缺铁性贫血患者补铁治疗3月后,再次检测血小板升高组患者血清EPO、TPO水平及血小板计数,与治疗前比较。结果:初诊时IDA伴血小板增多组患者血清EPO水平较血小板正常组及对照组显著升高,差异有统计学意义(P<0.05),IDA伴血小板正常组患者EPO水平轻度增高,与对照组之间比较,差异无统计学意义(P>0.05);IDA伴血小板增多组TPO水平轻度升高,与血小板正常组及对照组间比较,差异无统计学意义(P>0.05)。IDA伴血小板增多组经过3个月铁剂治疗,贫血纠正,血小板恢复正常,血清EPO、TPO水平与正常对照组比较,差异无统计学意义(P>0.05)。结论:缺铁性贫血(IDA)继发血小板增多患者,血清EPO水平明显升高,且与血小板增多密切相关;血清TPO水平只轻度升高;EPO与TPO二者之间可能存在协同作用导致血小板升高。Objective To detect serum EPO and TPO levels in patients of Iron deficiency anemia with secondary thrombocytopenia,and to study its clinical significance. Methods 122 cases with iron deficiency anemia were selected and divided into two groups according to the level of platelets. Forty healthy cases were designed as control group,ELISA was used to detect the serum levels of EPO and TPO. After the treatment of iron supplementation for three months,the serum levels of EPO and TPO and the platelet count were determined again. Result The serum levels of EPO in patients with thrombocytopenia were significantly higher than normal platelet count group and the control group（P〈0.05）; the level of EPO in patients with normal platelet count group was slightly higher,and there was no significant difference between the normal platelet count group and the control group（P〉0.05）. The TPO levels was no significant difference in the three groups（P〉0.05）.After patients anemia correction,platelet count reduced to normal level; the levels of serum EPO and TPO were no difference between patients group and control group（P〉0.05）. Conclusion In iron deficiency anemia（IDA） secondary thrombocytosis patients,the level of EPO is closely related with the platelet count,however,serum TPO level only increased slightly,there may be a synergistic effect between EPO and TPO to increase platelet count.
