初诊男性2型糖尿病肾病危险因素分析

摘　　要：目的探讨初诊男性2型糖尿病患者的肾病危险因素。方法收集我院85例男性初诊2型糖尿病患者的临床资料,分成3组:1组尿中无白蛋白或尿白蛋白<29 mg/d;2组尿白蛋白30~300 mg/d;3组尿白蛋白>300 mg/d,分析3组间临床特点间的差异。结果 3组患者中性粒细胞比例、淋巴细胞比例、血小板压积、尿酸水平差异有显著(P<0.05)。2组肌酐显著高于1组(P<0.05)。2组γ谷氨酰转肽酶显著高于其他2组(P<0.05)。3组间视网膜病变比例差异有统计学意义(P<0.05)。结论高肌酐、高尿酸等是糖尿病肾病的危险因素,中性粒细胞比例、淋巴细胞比例、血小板压积、胆固醇/甘油三酯等可预测糖尿病肾病的发生,糖尿病肾病与糖尿病视网膜病变平行相关。Objective The aim of this study was to investigate the risk factors of preliminary diagnosis male patients who had Type 2 Diabetes Mellitus （T2DM）.Methods The clinical characteristics of 85 preliminary diagnosis male patients with T2DM were collected. On the basis of the 24 hour urinary albumin,patients were dived into three groups,with the 24 hour urinary albumin lower than 29 mg,between 30 and 300 mg,higher than 300 mg respectively. Follow that the clinical character differences in different groups were analyzed. Results The percentage of neutrophilic granulocyte,the percentage of lymphocytes,thrombocytocrit and uric acid in three groups were statistically different （P〈0.05）.The creatinine in the second group was higher than that in the first group （P〈0.05）.The percentage of cholesterol on triglyceride in the second group was lower than that in the first group （P〈0.01）.The percentage of retinopathy in three groups was statistically different （P〈0.05）.Conclusion Higher level of creatinine and uric acid were the risk factors in diabetic nephropathy patients.The percentage of neutrophilic granulocyte,the percentage of lymphocytes, thrombocytocrit, the percentage of cholesterol on triglyceride may be used to predict diabetic nephropathy. The diabetic nephropathy was direct correlation with diabetic retinopathy.
