宫颈旁神经阻滞联合利多卡因凝胶用于宫腔镜手术麻醉效果观察

摘　　要：目的观察宫颈旁神经阻滞联合利多卡因凝胶黏膜表面浸润用于宫腔镜手术的麻醉效果。方法选择ASAⅠ或Ⅱ级择期宫腔镜手术患者60例,根据权重分配方法,将患者随机分为L组(异丙酚复合芬太尼)、H组(单行宫颈旁神经阻滞)、M组(宫颈旁神经阻滞联合利多卡因凝胶表面麻醉)各20例。连续监测3组心率(HR)、平均动脉压(MAP)、脉搏血氧饱和度(SpO_2),比较3组患者疼痛评分、不良手术并发症的发生率。结果术前3组血液动力学水平均较稳定,差异无统计学意义(P>0.05)。L组和M组患者在t_2、t_3时血流动力学稳定性优于H组(P<0.05)。H组疼痛评分明显高于L组和M组(P<0.05)。结论宫颈旁神经阻滞联合利多卡因凝胶用于宫腔镜手术麻醉安全、镇痛完全、易于观察和发现并发症。Objective To observe the application of cervical nerve block combined with lidocaine gel surface mucosal infiltration in the the anesthesia of hysteroscopy. Methods 60 cases of hysteroscopic ASA Ⅰ and Ⅱ patients undergoing elective surgery were selected. According to the weight allocation method,patients were randomly assigned to three different anesthesia methods three groups（ n = 20）,group L（ propofol combined with fentanyl）,group H（ cervical paravertebral nerve block）,group M（ paravertebral nerve block combined with lidocaine gel surface anesthesia）. To observe the heart rate（ HR）,mean arterial pressure（ MAP）,pulse oxygen saturation（ SPO2） of three groups. Compared the pain score of patients,adverse complications occurring probability of three groups. Results Preoperative 3 groups hemodynamic there was no statistic significantly（ P〉0. 05）. Time t2,t3,the hemodynamic stability of group L and group M was better than group H（ P〈0. 05）. The pain score of group H was higher than that of group L and group M（ P〈0. 05）. Conclusion Cervical nerve block combined with lidocaine gel for the safety of anesthesia,analgesia,easy to observe and find complications.
