乳山市30岁以上农村居民高血压综合防治效果评价

摘　　要：目的探索符合基层农村实际的可持续性高血压等慢性病综合防控模式。方法采取分层随机抽样方法,将2 689名30岁以上农村居民纳入干预队列,以合理膳食、适量运动、戒烟限酒、规范服药等为核心内容,对人群实施生活行为方式干预、高血压患者分级随访管理等综合措施,对干预效果进行对比评价。计数资料采用χ~2检验,计量资料采用方差分析。结果干预人群及高血压患者收缩压(Z=-6.07,P<0.01;Z=-14.66,P<0.01)和舒张压均值(Z=-4.43,P<0.01;Z=-10.85,P<0.01)较基线水平有不同程度下降,高血压患者知晓率、治疗率和控制率(χ~2=84.49、3.91、261.35,均P<0.05)明显高于基线水平,吸烟率由24.39%下降至20.68%,食盐摄入量由11.18g下降至10.09g,体育锻炼率由15.79%上升到19.42%。结论以综合防治措施开展人群高血压防治工作对逐步降低人群高血压患病率和提升高血压患者知晓率、治疗率、控制率具有明显成效。Objective To evaluate the effect of comprehensive intervention on hypertension control among residents from rural areas of Rushan,and to explore a control model for hypertension and other chronic diseases. Methods A stratified random sampling method was used to select 2 689 adult residents over the age of 30 from rural areas and a comprehensive intervention was introduced,including reasonable diet,regular exercise,quit smoking and limit alcohol consumption,standardized medication as core methods combined with life behavioral intervention and patients follow-up for a better management.The effect of intervention was evaluated. Results After the intervention,the mean systolic and diastolic blood pressure in normal subjects and in patients with hypertension both decreased significantly compared to the baseline levels（Z=-6.07,P〈0.01;Z=-14.66,P〈0.01）（Z=-4.43,P〈0.01;Z=-10.85,P〈0.01）.Meanwhile,the awareness rate,treatment rate and control rates of hypertension were significantly higher than that of baseline（χ~2=84.49,3.91,261.35,P〈0.05）.The percentage of smokers decreased to 20.68% from 24.39%,salt intake amount dropped from 11.18 g to 10.09 g,and 4% more residents started doing physical exercises. Conclusions A comprehensive intervention may gradually reduce the prevalence of hypertension and enhance the awareness rate,treatment rate and control rates of patients.
