黏液型铜绿假单胞菌在呼吸道感染患者中的年龄分布及药敏分析

摘　　要：目的分析呼吸道感染患者黏液型铜绿假单胞菌(PAE)的年龄分布及药敏结果 ,为临床抗感染治疗选药提供依据。方法收集40例(40株)住院患者合格呼吸道样本分离出的黏液型PAE进行纸片扩散法(K-B法)48 h读取药敏试验,另查LIS系统并进行统计。结果 40例呼吸道感染产黏液型PAE患者中≤49岁2例(5.0%),50~59岁4例(10.0%),60~69岁8例(20.0%),70~79岁11例(27.5%),≥80岁15例(37.5%)。≥60岁老年患者共有34例(85.0%)。且易复发感染;分离出黏液型PAE对多粘菌素、碳青霉烯类(亚胺培南、美罗培南)、哌拉西林/他唑巴坦、氨基糖苷类(庆大霉素、阿米卡星)、头孢类(头孢他啶、头孢吡肟)和哌拉西林抗菌药敏感性较高,均>80%。结论黏液型PAE是老年人呼吸道感染的重要影响因素,应加强对该菌的分离与鉴定,及时告知临床医生,为早期治疗控制感染提供依据,减少感染的复发。Objective To analyze age distribution and drug sensitivity outcome by mucoid pseudomonas aeruginosa（PAE） in respiratory tract infection patients. Methods A total of 40 cases（40 strains） mucoid PAE separated from qualified respiratory tract samples of hospitalized patients were collected for drug sensitivity test by Kirby-Bauer method（K-B method）. They were also taken into LIS system for test and summarization. Results Among 40 respiratory tract infection patients with mucoid PAE, there were 2 cases ≤49 years old（5.0%）, 4 cases in 50~59 years old（10.0%）, 8 cases in 60~69 years old（20.0%）, 11 cases in 70~79 years old（27.5%）, 15 cases ≥ 80 years old（37.5%）, and totally 34 cases ≥ 60 years old（85.0%） with easily recurrent infection. Separated mucoid PAE showed high sensitivity, as 80%, to polymyxins, carbapenems（imipenem, meropenem）, piperacillin/tazobactam, aminoglycosides（gentamicin, amikacin）, cephalosporin（ceftazidime, cefepime） and piperacillin. Conclusion As an important influencing factor on senile respiratory tract infection, mucoid PAE requires enhanced separation and identification, and timely notification to clinicians, in order to provide reference for early treatment and infection control, and to reduce recurrent infection.
