摘  要:目的探讨阴道宫颈分泌物中胎儿纤维连接蛋白(fFN)联合宫颈长度(CL)在双胎妊娠早产预测中的价值。方法 50例先兆早产的双胎妊娠孕妇为研究对象,检测孕妇阴道宫颈分泌物中fFN,B超测量CL,观察fFN、CL分别检测及联合检测预测双胎妊娠早产的敏感度、特异度和阳性预测值,分析其对早产的预测价值。结果 fFN、CL预测早产的敏感度(83.33%VS 85.71%)、特异度(71.05%VS 72.22%)和阳性预测值(47.62%VS 54.55%)比较,差异均无统计学意义(P>0.05)。fFN、CL预测早产的敏感度(83.33%、85.71%)与二者联合检测预测早产的敏感度(84.62%)比较,差异均无统计学意义(P>0.05);f FN、CL预测早产的特异度(71.05%、72.22%)和阳性预测值(47.62%、54.55%)均低于二者联合检测预测早产的特异度(97.30%)和阳性预测值(91.67%),差异均具有统计学意义(P<0.05)。结论双胎妊娠中联合阴道宫颈分泌物fFN和CL测量,增加了其阳性预测值和特异度,对双胎妊娠早产有较好的预测意义,值得推广。Objective To explore the value of cervicovaginal secretions fetal fibronectin(fFN) combined with cervical length(CL) in prediction of premature delivery in gemellary pregnancy. Methods A total of 50 gemellary pregnancy women in threatened premature labor as study subjects, their cervicovaginal secretions fFN were detected, and CL was measured by B ultrasound. Observation were made on sensitiveness, specificity and positive predictive value of premature delivery gemellary pregnancy by fFN, CL respective detection and combined detection, and their predictive value for premature delivery was analyzed. Results fFN and CL had no statistically significant difference in sensitiveness(83.33% VS 85.71%), specificity(71.05% VS 72.22%) and positive predictive value(47.62% VS 54.55%) of premature delivery prediction(P〉0.05). fFN and CL had no statistically significant difference in sensitiveness of premature delivery prediction(83.33%, 85.71%) comparing with combined detection(84.62%)(P〉0.05). fFN and CL had lower specificity(71.05%, 72.22%) and positive predictive value(47.62%, 54.55%) than 97.30% and 91.67% in combined detection, and their difference had statistical significance(P〈0.05). Conclusion Combination of cervicovaginal secretions fFN and CL measurement in gemellary pregnancy increases its positive predictive value and specificity, and shows good predictive significance for premature delivery in gemellary pregnancy. It is worth promoting.
