新疆铁路机车乘务人员高血压患病情况及相关危险因素

摘　　要：目的了解新疆铁路机车乘务人员高血压患病情况,并分析其相关危险因素,为乘务人员高血压的预防提供参考依据。方法选取新疆乌鲁木齐铁路局机务段239名乘务人员,通过血压测量,计算其高血压患病情况,并运用单因素分析,观察年龄、工作时间、工龄、个人习惯等一般资料对高血压患病情况的影响,将存在统计学差异的因素纳入Logistic多因素回归分析,总结影响新疆铁路机车乘务人员高血压患病情况的危险因素。结果 239名受试者中,共有57名(23.85%)检出高血压,其中1级(轻度)高血压29例(50.88%),2级(中度)高血压25例(43.86%),3级(重度)高血压3例(5.26%)。多因素分析显示,年龄、工龄、劳动强度、职业紧张评分是影响新疆铁路机车乘务人员高血压患病的独立危险因素,睡眠长度、工作中按时饮食为保护性因素(P<0.05)。结论新疆铁路机车乘务人员高血压患病率较高且以轻、中度高血压为主,其患病危险因素与生活习惯不佳有关。Objective To analyze the prevalence of hypertension and related risk factors of Xinjiang railway locomotive crew, and provide some references for the prevention of hypertension of locomotive crew. Methods 239 crew members in Urumqi Xinjiang Railway Bureau were involved in the study. The prevalence of hypertension was calculated through the measurement of blood pressure of each member. The clinical data such as age, working hours, length of service, personal habit was involved in the univariate analysis on prevalence of hypertension. The factors with statistical difference were involved in the Logistic regression analysis, and the risk factors which impacted the prevalence of hypertension of locomotive crew of Xinjiang railway were summarized. Results 57 cases of 239 participants were diagnosed with hypertension, with the incidence rate of 23.85% ; among which, 29 cases （50.88%） were grade- 1 （mild） hypertension, 25 cases （43.86%） were grade- 2 （moderate） hypertension, and 3 cases （5.26%） were grade- 3 （severe） hypertension. Multivariate analysis showed that age, length of service, labor intensity, occupational stress scores were the independent risk factors for the prevalence of hypertension of locomotive crew in Xinjiang railway, length of sleep, regular diet at working time were protective factors （P〈0.05）. Conclusion The prevalence rate of hypertension of Xinjiang railway locomotive crew is high, which is mainly consisted with mild and moderate hypertension, and poor living habits is the risk factor for illness.
