椎间孔镜TESSYS技术治疗腰椎间盘突出症的临床研究

摘　　要：目的研究探讨椎间孔镜TESSYS(transforaminal endoscopic spine system,经椎间孔进入椎管内行直接神经根松解和减压的技术)在腰椎间盘突出症(1umbar disc herniation,LDH)治疗中的临床应用疗效及安全性。方法选取于2014年01月-2015年12月于我院诊治的50例腰椎间盘突出症的患者,所有患者在入院确诊后均予以经皮椎间孔镜TESSYS技术治疗,治疗后分别观察与比较治疗前、治疗后1个月、治疗后6个月、治疗后12个月患者的腰部疼痛及下肢疼痛的疼痛视觉模拟评分(VAS)及功能障碍指数评分(ODI),并在治疗后12个月时应用MacNab临床疗效评定标准判定治疗优良率。结果与治疗前比较,患者在治疗后1个月、治疗后6个月、治疗后12个月时其腰部疼痛及下肢疼痛的疼痛视觉模拟评分及功能障碍指数评分均较治疗前有显著下降,其差异具有统计学意义(P<0.05);与治疗后1个月比较,患者在治疗后6个月、治疗后12个月时其腰部疼痛及下肢疼痛的疼痛视觉模拟评分及功能障碍指数评分均有显著下降,其差异具有统计学意义(p<0.05);而治疗后6个月与治疗后12个月时比较,其腰部疼痛及下肢疼痛的疼痛视觉模拟评分及功能障碍指数评分未有显著性差异,不具有统计学意义(P>0.05);治疗后12个月时不同节段的腰椎间盘突出症患者其腰部疼痛及下肢疼痛的疼痛视觉模拟评分及功能障碍指数评分均无显著性差异,不具有统计学意义(P>0.05);治疗后判定患者治疗优良率达90.00%。结论在腰椎间盘突出症的治疗中,运用椎间孔镜TESSYS技术,具有良好的治疗效果和安全性,值得临床上进一步应用与推广。Objective To investigate the clinical efficacy and safety of TESSYS in the treatment of lumbar disc herniation.Methods SO cases of patients with LDH treated in our hospital from January 2014 to December 2015 were selected.All patients were treated with TESSYS after admission to hospital.Patients＇ visual analog pain scale（VAS） and Oswestry Disability Index（ODI） in the waist and lower limb were observed and compared respectively before treatment,1 month,6 months and 12 months after treatment.Clinical efficacy evaluation criteria of MacNab were used to determine the good rate of treatment at 12 months after treatment.Results The patients＇ VAS and ODI in the lumbar and lower limb at 1 month,6 months and 12 months after treatment were obviously lower than those before treatment,the difference had statistical significance（P〈0.05）;patients＇ VAS and ODI in the lumbar and lower limb at 6 months,12 months after treatment had a significant decrease compared with those at 1 month after treatment,the difference was statistically significant（P〈0.05）;The VAS and ODI in the lumbar and lower limb at 12 months after treatment had no remarkable difference compared with those at 6 months after treatment,which had no statistical significance（P〉0.05）;The good rate of treatment was 90.00%after treatment Conclusion The use of TESSYS in the treatment of LDH has good therapeutic effect and safety,it is worthy of further clinical application and promotion.
