化学发光法和胶体金法检测HIV抗体与免疫印迹法确诊结果分析

摘　　要：目的通过调查医院就诊患者HIV抗体阳性情况,为医院院内感染与医护人员防护工作提供重要依据。方法采用化学发光法(CLIA)与胶体金法对2013~2015年入院患者输血前HIV抗体筛查检测,筛查阳性标本送陕西省疾病控制中心用重组免疫印迹法(WB法)进行确诊。结果化学发光法HIV筛查阳性标本85例,胶体金法筛查阳性标本47例,经重组免疫印迹法最终确证59例,CLIA法阳性符合率69.4%(59/85),胶体金法的阳性符合率79.6%(47/59),用CLIA法,样本测定值/临界值(S/CO)在1.0~5.0 13例,经WB法确证1例,不确定5例,S/CO在5.0~10.0 23例,确证9例,不确定4例,S/CO值在10.0~29.5的49例,则经过WB法确证49例随着S/CO值的升高,胶体金法的阳性率随之升高反之阳性率越低。结论 CLIA法抗HIV抗体筛查检测灵敏度高、特异性强、自动化程度高,可用于临床批量检测HIV抗体筛查。胶体金法操作简单,检测时间快、保存携带方便,可于急诊等快速检测中。Objective Through the investigation of hospital patients with HIV antibody positive situation for the hospital hospital infection and health care personnel to provide an important basis for prevention work.Methods HIV antibody screening was performed in patients admitted to hospital from 2013 to 2015 by chemiluminescence method and the colloidal gold method.Screened positive samples were confirmed by recombinant western blotting（WB） at Shaanxi Provincial Center for Disease Control and Prevention.Results 85 cases were positive for HIV screening by chemiluminescence method,47 cases were positive for coliform gold screening method,and 59 cases were confirmed by WB.The positive rate of CLIA method was 69.4%（59/85）,and colloidal gold method was 79.6%（47/59）.For the CLIA method,13 cases of sample measured value/critical value（S/CO） were between 1.0 and 5.0,confirmed by WB method in 1 case,and 5 cases of uncertainty; 23 cases of S/CO between 5.0 and 10.0,confirmed in9 cases,and 4 cases of uncertainty,49 cases of S/CO value of 10.0 ~ 29.5,then confirmed by WB for 49 cases.It was found that the positive rate of colloidal gold method increased with the increase of S/CO value.Conclusion CLIA anti-HIV antibody screening detection of high sensitivity,specificity,high degree of automation,can be used for clinical batch testing of HIV antibody screening.Colloidal gold method is simple,fast detection time,save easy to carry,can be quickly detected in the emergency and other.
