血清人附睾蛋白4表达与慢性肾脏病的相关性研究

摘　　要：目的初步探讨人附睾蛋白4(human epididymal protein 4,HE4)在慢性肾脏病(chronic kidney disease,CKD)中的表达水平及临床诊断价值。方法收集2014年4月~2015年12月南通大学附属医院确诊为CKD的92例非肿瘤患者及84例健康对照者的血清样本,分别检测尿素氮(blood urea nitrogen,BUN)、血清肌酐(creatinine,Scr)、胱抑素C(cystatin C,Cys C)和β2微球蛋白(β2microglobulin,β2MG)在不同CKD分期中的水平变化,以及HE4浓度与BUN,Scr,Cys C和β2MG浓度的相关性。用ROC曲线评价HE4,BUN,Scr,Cys C和β2MG这五个指标对CKD辅助诊断价值。结果肾功能受损的患者血清HE4表达量为388.2(130.1~1 659.5)mlu/L,明显高于对照组的38.1(32.77~48.17)mlu/L,(F=16.237,P=0.001)。随着肾损害的加重患者血清HE4水平也随之逐渐升高,各组间差异有统计学意义(P<0.05)。HE4水平与Cys C,132MG水平呈正相关,用ROC曲线下面积(area under the curve,AUC)分析血清HE4,BUN,Scr,Cys C和β2MG检测对CKD的诊断效能,结果显示曲线下面积分别为0.878,0.785,0.816,9.874和0.819,HE4有明显的诊断价值。结论 HE4检测时要考虑肾功能受损害的存在,HE4有可能是一种新的CKD早期诊断指标。Objective To investigate the expression of human epididymal protein 4 （HE4） in chronic kidney disease （CKD） and its clinical value. Methods From April 2014 to December 2015, Serum samples of 92 non-cancer patients diagnosed as CKD and 84 healthy controls were collected in Nantong University Hospital. HE4,BUN,Scr,Cys C and 92MG were detec- ted. The difference of HE4 expression in different stages of CKD and the correlation of HE4 with Urea, Scr, Cys C and β2MG were analysized, respectively. ROC curve was used to evaluate the auxiliary diagnostic value of HE4, BUN, Scr,Cys C and β2MG. Results The serum HE4 expression in patients with impaired renal function was 388.2 （130.1~1 659.5）mIu/ L,（F= 16. 237,P=0. 001）. Which was significantly higher than that in the control group [38.1（32.77~48.17）mIu/L]. The serum HE4 expression was increased by the stage of renal damage and the difference was existed among different groups （P〈0. 05）. HE4 expression was positive related with Cys C and β2MG. The AUC of HE4, BUN,Scr, Cys C and β2MC- were 0. 878,0. 785,0. 816,0. 874 and 0. 819,respectively. Conlusion It needs to consider the exsits of impaired renal function when the HE4 was detected. The HE4 might be a novel early diagnostic indication for CKD.
