子宫内膜液基细胞学检查在子宫内膜癌筛查中的效果研究

摘　　要：目的探讨子宫内膜液基细胞学检查在子宫内膜癌筛查中的效果。方法对2014年1月-12月就诊于上海市浦东新区浦南医院的150例具有诊断性刮宫或宫腔镜活检的患者,应用宫腔刷刷取子宫内膜进行液基细胞学诊断,同时行诊断性刮宫或宫腔镜活检进行病理诊断。以诊断性刮宫或宫腔镜活检的病理诊断结果作为诊断"金标准",分析子宫内膜液基细胞学诊断的诊断符合率、敏感度及特异度。结果 150例患者中,子宫内膜癌前病变13例,子宫内膜癌12例;宫腔刷采集子宫内膜进行液基细胞学诊断的诊断符合率为96.0%,敏感度95.2%,特异度96.9%。宫腔刷不仅可用于液基细胞学诊断,同时对刷取的子宫内膜组织物也可进行组织学诊断。结论宫腔刷刷取子宫内膜进行液基细胞学检查具有较高的诊断符合率、敏感度及特异度;宫腔刷刷取的组织物组织学诊断有望为子宫内膜癌的筛查提供组织学确诊依据。Objective To explore the effect of endometrial Thinprep cytology test（ TCT） in endometrial cancer screening.Methods A total of 150 patients receiving diagnostic curettage or hysteroscopic biopsy in the hospital from January to December in 2014 were selected,endometrial TCT was conducted by intrauterine brush,pathological diagnosis after diagnostic curettage or hysteroscopic biopsy was obtained. Taking the result of pathological diagnosis as gold standard,the coincidence rate,sensitivity,and specificity of TCT were analyzed. Results Among 150 patients,thirteen patients were diagnosed as endometrial precancerous lesion,and twelve patients were diagnosed as endometrial cancer. The coincidence rate,sensitivity,and specificity of TCT were 96. 0%,95. 2%,and 96. 9%,respectively. Intrauterine brush not only can be used for TCT,but also conduct histologic diagnosis. Conclusion The coincidence rate,sensitivity,and specificity of TCT by intrauterine brush are high. The histologic examination is expected to provide a basis for histologic diagnosis in endometrial cancer screening.
