早孕期胎盘体积及血管化参数预测在复发性流产中的价值

摘　　要：目的探讨早孕期胎盘容积及其血管化参数预测复发性流产者妊娠结局的价值。方法将优生遗传门诊就诊同时自愿进行三维能量多普勒超声测量的孕妇51例分为两组,研究组30例为复发性流产孕妇,对照组21例为正常妊娠孕妇,在早孕期使用三维能量多普勒超声胎盘容积及VI、FI、VFI测量,将两组孕妇所得的数据进行统计学分析比较。结果观察组孕妇早孕期胎盘容积及血管化指数VI、VFI显著低于对照组孕妇,差异有统计学意义(P<0.05),两组孕妇FI差异无统计学意义(P>0.05)。妊娠早期胎盘容积、胎盘血流VI、FI、VFI做为预测持续妊娠≥12周指标的ROC曲线下面积分别为0.771、0.907、0.592、0.771,FI的预测准确度较低,胎盘容积、VFI有一定准确度,VI有较高准确度。结论早孕期胎盘的VI对预测复发性流产者妊娠结局最为敏感,三维能量多普勒在复发性流产监测中有较好的应用价值。Objective To explore the predictive value of placental volume and vascular parameters during the first trimester of pregnancy for pregnancy outcomes of patients with recurrent spontaneous abortion（ RSA）. Methods Fifty-one pregnant women receiving threedimensional power Doppler ultrasonography voluntarily in Department of Eugenics and Heredity were divided into study group（ including 30 cases with RSA） and control group（ including 21 normal pregnant women）. Three-dimensional power doppler ultrasound was used to detect placental volume,vascularity index（ VI）,farbenidox index（ FI）,and vascular arbenidox（ VFI） during the first trimester of pregnancy. The data was statistically analyzed and compared between the two groups. Results Placental volume,VI,VFI during the first trimester of pregnancy in study group were statistically significantly higher than those in control group（ P〈0. 05）,there was no statistically significant difference in FI between study group and control group（ P〉0. 05）. The areas under ROC curve of placental volume,VI,FI,and VFI in predicting ongoing pregnancy≥12 weeks were 0. 771,0. 907,0. 592,and 0. 771,respectively,the accuracy of FI was relatively low,followed by placental volume and VFI,and the accuracy of VI was high. Conclusion In all placental vascular parameters,VI as a predicter for pregnancy outcome of RSA is more sensitive than the others. Three-dimensional power Doppler ultrasound has a good application value in monitoring of RSA.
