子宫动脉灌注化疗栓塞术后期待治疗在剖宫产瘢痕妊娠的应用研究

摘　　要：目的探讨子宫动脉灌注化疗栓塞术后期待治疗应用于剖宫产瘢痕妊娠(cesarean scar pregnancy,CSP)的疗效和安全性。方法选取2007年1月至2014年7月重庆三峡中心医院江南分院诊治的80例外向型CSP患者的病历资料,根据治疗方案选择的不同将子宫动脉灌注化疗栓塞术后期待治疗(不清宫)的40例作为研究组,子宫动脉灌注化疗栓塞联合彩超监测下清宫治疗的40例为对照组,对比分析两组治疗的有效性和安全性。结果两组治疗有效性比较差异无统计学意义(P>0.05)。研究组的阴道出血量、平均住院时间明显少于对照组(P<0.05)。结论子宫动脉灌注化疗栓塞术后期待治疗CSP,具有出血量少、住院时间短、治疗效果明显等优点,避免了清宫带来的大出血、子宫穿孔等风险,减轻了患者痛苦和经济负担,值得临床推广。Objective To investigate the efficacy and safety of expectant treatment after uterine arterial infusion chemoembolization in treating cesarean scar pregnancy（CSP）. Methods The medical records of 80 patients with extroverted CSP were collected from January 2007 to July 2014 in Jiangnan Branch, Central Hospital of Chongqing Three Gorges and divided into study group and control group according to treatment options. The study group （40 cases ） used expectant treatment after uterine arterial infusion chemoembolization,while the control group used uterine arterial infusion chemoembolization combined with color Doppler ultrasound monitoring curettage. The efficacy and safety of the two groups were compared and analyzed. Results There was no significant difference in the efficacy of the two groups（P 〉0. 05 ）. The amount of vaginal bleeding and the average length of hospital stay in the study group were significantly lower than those in the control group（ P 〈 0.05 ）. Conclusion Expectant treatment for CSP after uterine arterial infusion chemoembolization reduces the palace steps, with less bleeding, shorter hospitalization time, obvious treatment effect etc. It avoids massive hemorrhage, uterine perforation due to curettage. It is worthy of clinical application and promotion in order to reduce the patient＇s pain and financial burden.
