糖尿病足患者住院费用调查及影响因素的研究

摘　　要：目的调查糖尿病足(DF)患者住院费用,分析其影响因素。方法对2009~2015年901例、1146例次DF患者进行回顾性调查,分析住院时间、住院费用及构成等。结果 2015年平均发病年龄较2009年减少5.7岁(x^2=45.89,P<0.001)。糖尿病合并症及并发症、Wagner分级、年龄对患者住院时间及费用均有影响(P<0.05)。消费价格指数经调整后,住院费用先增长后下降,以2012年最多(27629.6元);住院时间整体呈下降趋势,2015年较2009年减少15.5 d(x^2=61.16,P<0.001);患者合并多种并发症,且并发症患病率逐年增加。结论 2009~2015年,DF患者的医疗费用先增长后下降,住院时间明显减少;发病人群以男性居多,且具有高龄化、病程长、高费用等特点。糖尿病患者应及早采取措施,严格控制血糖,降低糖尿病并发症,预防DF的发生。Objective To investigate the hospitalization expenses of diabetic foot and its influencing factors. Methods The data of diabetic foot from 2009 tO 2015 were retrospectively analyzed according to a unified protocol, including hospital days, costs and composition. Results 901 patients （1146 admissions）with diabetic foot were included in the analysis. The average age of diabetic foot in 2015 is younger 5.7 years than in 2009 （X2 = 45.89, P〈0. 001）. The diabetic complications, Wagner grades and age affected their hospital days and cost （P〈0.05）. After adjustment for consumer price index （CPI）, the total cost of hospitalization increased to the highest in 2012 （27,629.6 RMB） and then declined. Hospital days decreased in 2015 with I5.5 days less than in 2009 （x2= 61.16, P〈0. 001）. The prevalence of complications increased year by year. Conclusion The total cost of diabetic foot has a trend of increase first and then decline in seven years. The hospitalization time has a trend of decrease. Male, aging, long course and high cost are the characteristics of diabetic foot patients. Diabetic patients should take measures to strictly control blood sugar and to reduce the prevalence of diabetic complications.
