摘  要:目的:分析DAMIS全髋关节置换术中股外侧皮神经损伤的发生率及其对患者术后髋关节功能恢复及生活质量的影响。方法:在2013年1月至2016年4月采用DAMIS完成的初次单侧全髋关节置换术的病例中,按纳入及排除标准选出43例(43髋)术后有股外侧皮神经(LFCN)损伤症状的病例为损伤组,同时随机选取50例(50髋)术后无股外侧皮神经损伤症状的病例为未损伤组,进行回顾性分析。观察两组手术时切口长度、手术时间、住院天数等,以Harris评分评估髋关节功能,以生活质量评分表(SF-36)评估生活质量,并对两组调查结果进行对比研究。结果:93例(93髋)均获得随访,平均随访32.5个月(6~44个月),两组手术时切口长度、手术时间、住院天数及Harris评分均无统计学意义(P>0.05);未损伤组SF-36总评分(112.14±3.23)分高于损伤组(105.86±3.37)分(P<0.05)。本组病例LFCN损伤主要症状包括感觉迟钝(13例,30.2%)、疼痛感(20例,46.5%)、刺痛感(10例,23.3%),症状有自然缓解的29例(67.4%)。结论:LFCN损伤对患者髋关节功能的恢复最终结果没有影响,但是LFCN损伤症状将降低接受了DAMIS THA患者的生活质量。Objective:To analyze the incidence rate of LFCN injury and effect of LFCN injury in DAMIS THA for hip joint function recovery and quality of life afer DAMIS THA. Methods: Retrospectively analyzed 93 patients (93 hips) who ac- cepted DAMIS THA from Janury, 2013 to May, 2016. Length of incision, operative time, blood loss, hospital stay, Harris scores and SF-36 scores were observed. Comparison of difference between injury group and uninjury group was made. Resuits : All 93 patient were followed for average 32.5 months (6 - 44 months). For the involed patients ,43 cases were in injury group,average(68.84 ± 8.70) years old,50 cases were in uninjury group, average(69.72 ± 7.22) years old. The SF-36 scores in the uninjury group ( 112.14± 3.23 ) were higher than that of the injury group ( 105.86 ±3.37 ). The main symptoms of injury group included hypoesthesia, pain, jolt-like sensation. 29 cases had spontaneous resolution. Conclusion:The LFCN injury does not affect recovery of hip joint function, but reduced the life quality of patients after DAMIS THA.
