摘　　要：目的比较腹股沟疝术前预防用药与术后用药对术后感染的疗效。方法选取医院外科2013年8月至2016年8月收治的腹股沟疝患者60例,采用平行随机抽样法分为术后干预组与术前预防组,各30例。术后干预组在术后1~2 d给予静脉滴注头孢孟多,术前预防组在术前2 h给予静脉滴注头孢孟多,对比两组患者的手术时间、住院时间、抗菌药物使用费用及术后感染发生情况。结果术前预防组的抗菌药物使用费用明显低于术后干预组,差异有统计学意义(P<0.05);术前预防组术后感染发生率为3.33%,明显低于术后干预组的16.67%,差异有统计学意义(P<0.05)。结论腹股沟疝术前预防性使用抗菌药物可明显降低术后感染发生率,减少药物使用费用,是抗菌药物应用的最佳时机。Objective To investigate the infection prevention efficacy in the prevention of postoperative infection on patients with inguinal hernia after surgery. Methods 60 cases of inguinal hernia treated in the hospital from August 2013 to August 2016 were selected and divided into the preoperative intervention group and postoperative intervention group according to the parallel random sampling method, 30 cases in each group. The postoperative intervention group received conventional antibiotic therapy （intravenous cefamandole）in postoperative 1-2 d;the preoperative prevention group was given prophylactic antibiotic therapy （intravenous cefamandole）in the preoperative 2 h . The operation time ,hospital time ,antibiotics cost and postoperative infection were compared between the two groups . Results The cost of antimicrobial use in preoperative intervention group was significantly lower than that in postoperative intervention group （P 〈 0. 05）. The incidence rate of postoperative infection in the preoperative intervention group was 3. 33%,which was significantly lower than 16. 67% in the postoperative intervention group （P 〈 0. 05）. Conclusion Prophylactic antibiotic treatment in inguinal hernia before operation can significantly reduce the incidence of postoperative infection,reduce the cost of the use of antibiotics, and is the best timing for antibiotic application.
