摘  要:目的通过调查多药耐药菌(MDRO)的分布与流行特点,为有效降低MDRO医院感染提供临床依据。方法干预前资料来自回顾性收集2015年MDRO数据;干预后资料为采用目标性监测后2016年MDRO监测资料,对比评价防控及干预措施落实效果;资料采用SPSS 16.0软件进行统计分析。结果 MDRO发现率由2015年的0.3%降至2016年0.25%,医院MDRO感染率由2015年的0.15%降至2016年0.11%,差异有统计学意义(P<0.05);目标性监测期间隔离医嘱、隔离标识、医疗废物处理、宣教等较2015年改善显著,防控措施落实有效(P<0.05)。结论掌握MDRO的耐药特点及分布,加强防控措施的监管,查找防控薄弱环节,是有效预防或降低MDRO医院感染的关键;抗菌药物合理应用是下一步目标性监测督导重点。OBJECTIVE To investigate the distribution and epidemic characteristics of multidrug-resistant organisms(MDRO),so as to provide clinical basis for effectively reducing MDRO infection.METHODS Pre-intervention data were obtained from MDRO data from Jan.to Dec.2015 via retrospective study,and post-intervention data were obtained from MDRO data from Jan.to Dec.2016 after targeted surveillance.The implementation effects of prevention and control and intervention measures were comparatively evaluated.The data were statistically analyzed using SPSS 16.0software.RESULTS The MDRO detection rate was reduced from 0.30%in 2015 to 0.25% in2016.The detection rate of nosocomial infection was reduced from 0.15%in 2015 to 0.11%in 2016.The differences between before and after intervention were all significantly different(P〈0.05).The quarantine order,signs,medical waste treatment,and propaganda and education in the targeted surveillance had significantly improved in the year of 2016 compared to 2015(P〈0.05).Prevention and control measures were efficient.CONCLUSION The key points for prevention and reducing the nosocomial infection of MDRO are to know the drug resistance characteristics and distribution of MDRO,strength supervision and control measures,and find out the weakness of prevention and control measures.The rational use of antibacterial drugs would be our next supervision focus of the targeted surveillance.
