不同地区医疗机构医务人员安全注射现状调查研究

摘　　要：目的探讨甘肃省不同地区医疗卫生机构安全注射管理情况,为改善医务人员锐器伤现状提供科学依据。方法采用多阶段分层整群随机抽样方法,根据地理特征选取甘肃省28所医院作为调查对象,对2016年其安全注射管理情况进行问卷调查,调查结果进行分析。结果被调查的28所医疗机构中,有25所明确设立安全注射的管理部门,仅3所医疗机构未设立;其中,39.29%医疗机构负责医务人员安全注射的管理部门为护理部;有22所占78.57%组织(参加)安全注射培训,其中市级比例为83.33%,县级医疗机构为77.27%;有21所不同级别医疗机构对针刺伤造成的额外医疗费用有减免政策,其中市级和县级医疗机构分别占83.33%和72.72%;仅5所医疗机构按照《阻断院感注射传播,让注射更安全(2015-2018年)》专项工作指导方案开展了培训、考试、督查、考核等安全注射专项工作,其余医院均未开展。结论甘肃省各医疗机构安全注射管理工作不容乐观,遵循区域特色,建立健全管理组织,积极开展有效培训,建立完善管理机制,是提高医疗机构安全注射的有效途径。OBJECTIVE To explore the current status of safe injection management among medical institutions in different regions of Gansu province, so as to provide scientific basis for improving the status of sharp injury of medical staff. METHODS According to geographic feature, 28 medical situations were randomly selected to inves- tigate the safe injection in 2016 by stratified multi--stage cluster sampling method. The survey was conducted on the safe injection management in 2016, and the results were analyzed. RESULTS Twenty-five medical institutions established management department of safe injection and 3 medical institutions did not establish management de- partment of safe injection among 28 medical institutions. Totally 39.29% of management departments of safe in- jection were nursing department In addition, 22 medical institutions （78. 57%） organized （or participated in） training of safe injection, including 83.33% of municipal medical institutions and 77.27 M of county medical insti- tutions. Also, 21 different levels of medical institutions （75.00%） had policies of extra medical cost reduction on sharp injury and the rates of municipal and county medical institutions were 83.33% and 72.72 M. There were only 5 medical institutions that carried out the training, examination, supervise, evaluation and other work of safe in- jection according to the ＂ Prevent the spread of hospital injection and let injection safer （2015- 2018）＂. CONCLUSION The current situations of medical institutions on safe injection in Gansu province are not very good. So the effective ways of improving safe injection among medical institutions are to establish and perfect the man- agement organization, actively carry out effective training and establish a perfect management mechanism following the regional characteristics.
