摘  要:目的探讨骨端骨折CT容积再现关节分拆后关节面损伤的3D表现形式及其意义。方法对99例骨端骨折患者进行CT扫描,分别进行"CT容积再现常规法"(VR常规法)及"CT容积再现关节分拆法"(VR关节分拆法)后处理,对二组图像在骨折关节面损伤的显示效果进行比较观察与分析。结果 99例骨端骨折中VR关节分拆后3D图像能直观、充分显示骨端骨折关节面损伤全貌。其主要CT表现特征为:关节面裂隙征;关节内游离碎骨片及关节面骨质缺损征;关节内撕脱分离征;关节面龟裂征;关节面碎裂错位征;关节面爆裂分离变形征。结论 VR关节分拆后3D彩色图像能直观逼真显示骨端骨折关节面损伤严重程度和细节,对骨端骨折治疗方案选择有帮助。Objective To study the 3D manifestation and clinical value of joint surface injury after joint disassembly under the computed tomography (CT) volume rendering for extremities fractures patients. Methods Ninety-nine extremities fracture patients were taken "CT-VR conventional method" and "CT-VR joint disassembly method" after CT scan,respectively. The results of joint surface injury were compared and analyzed between two groups. Results For all 99 cases, images by using "CT-VR joint disassembly method" could display full extremities fracture surface injury directly and completely. The main CT features of manifestation were as below., feature of single joint surface crack, feature of intra-articular dissociable bone chips and joint surface bony defect, feature of intra-articular avulsion-diastasis, feature of several joint surface cracks, feature of joint surface fragmenting and mismatch; feature of joint surface fragmenting and separate deformation. Conclusion The colorful 3D images after post process by "CT-VR joint disassembly method" can show severity and specified details of extremities fracture joint surface injury exactly. It is useful references to determina- tion treatment options for an extremities fracture.
