前路锁定钢板联合加压螺钉治疗踝关节创伤性关节炎的初期疗效

摘　　要：目的探讨前路锁定钢板联合加压螺钉行踝关节融合治疗创伤性踝关节炎的临床疗效。方法 2012-06/2015-09月收治的23例创伤性踝关节炎患者的临床资料进行回顾性分析,评价其关节愈合时间及愈合率;记录视觉模拟评分(visual analogue scale,VAS);运用美国足踝协会(American orthopedic foot and ankle society,AOFAS)评分系统评价足踝功能;记录术中及术后并发症;询问患者的整体满意度。结果术后随访12~51个月,平均35.4个月,所有关节在16周内都获得骨性融合;VAS疼痛评分显著降低;AOFAS评分从术前(30.8±8.5)分增加到术后(82.4±4.8)分;患者整体满意度91.30%。结论前路锁定钢板联合加压螺钉行踝关节融合术治疗终末期创伤性踝关节炎操作简单有效,可显著缓解疼痛、改善终末期踝关节炎患者的下肢功能。Objective To investigate-the short-term clinical efficacy of anterior locking plate plus compression screw for post-traumatic ankle arthritis. Methods The clinical data of 23 post-traumatic ankle arthritis patients were retrospectively analyzed from June 2012 to September 2015. The healing time and rates of joint were evaluated, visual analogue scale （VAS） was recorded, the ankle functions were assessed by American orthopedic foot and ankle society （AOFAS）, intraoperative and postoperative complications were recorded, and overall satisfaction of patients were researched. Results The post-operation follow-up time were average 35.4 months （ranged, 12-51 months）. All joint were showed bone fusion among 16 weeks. VAS pain scores were significantly decreased. The mean AOFAS scores were improved from preoperative （30. 8 ± 8. 5） scores to postoperative （82. 4 ± 4. 8） scores. Patients＇ overall satisfaction rate was 91.3%. Conclusion Ankle Arthrodesis by anterior locking plate plus compression screw is simple, effective, remarkablely relieves pain, and improves lower limb function for post-traumatic ankle arthritis.
