针药结合治疗糖尿病周围神经病变临床研究

摘　　要：目的:探讨针药结合治疗阴虚血瘀型糖尿病周围神经病变(diabetes mellitus peripheral neuopathy,DPN)的临床疗效。方法:纳入DPN患者78例,随机分为对照组和治疗组,每组各39例。两组均给予控制饮食等基础治疗,对照组配合硫辛酸注射液治疗,治疗组在对照组基础上予以滋阴活血柔肝汤口服配合针刺干预,共治疗4周。比较治疗后两组患者中医症状评分、神经传导速度及临床疗效。检测两组血液流变学水平。结果:治疗后,治疗组的中医症状指标评分显著低于对照组,腓总神经和正中神经的感觉神经传导速度和运动神经传导速度明显高于对照组,差异有统计学意义(P<0.01);治疗组的有效率为92.31%,显著高于对照组的71.79%(P<0.05);治疗组治疗后患者的血液流变学水平改善情况显著优于对照组,差异有统计学意义(P<0.01)。结论:滋阴活血柔肝汤联合针刺治疗阴虚血瘀型DPN,能够显著改善患者症状、神经传导速度及血液流变学水平,提高临床疗效。Objective：To investigate the clinical effect of acupuncture combined with medicine in treating diabetes mellitus peripheral neuopathy（DPN） with Yin deficiency and blood stasis syndrome.Methods：78 patients with DPN were randomly divided into the treatment group and the control group of 39 cases in each group.Both groups received basic treatment with alimentary control and so on.Cases of control group took thioctic acid injection.Based on control group,cases of treatment group were given with Ziyin Huoxue Rougan decoction and acupuncture.4 weeks were used to observe treatment effect.Scores of TCM symptoms,nerve conduction velocity,and efficacy were compared between two groups.Hemorheology level was detected in the two groups.Results：After treatment,scores of TCM symptoms were remarkably lower,while than those of contol group（P 0.01）.Nerve conduction velocity of SNCV and MNCV was higher than control（P 0.01）.The effective rate of treatment group was 92.31%,which was superior to 71.79% of the control group（P 0.05）.Levels of hemorheology indexes of treatment groups were evidently lower than treatment group after treatment（P 0.01）.Conclusion：Efficacy of Ziyin Huoxue Rougan decoction combined with acupuncture in treating DPN can improve symptoms and nerve conduction velocity and hemorheology level,and enhance the clinical effect.
