摘  要:目的综合评价中国成年女性雌激素受体α基因(Estrogen Receptorαgene,ESR1)PvuⅡ(T/C)和XbaI(A/G)位点多态性与骨密度(Bone Mineral Density,BMD)的关系。方法采用Meta分析的方法对1994年1月-2016年6月国内外公开发表的关于中国成年女性ESR1基因Pvu II(T/C)和XbaI(A/G)位点多态性与BMD关系的研究进行综合定量分析。结果 15篇文献,共4 753名中国成年女性被纳入。Meta分析显示,不同PvuⅡ(T/C)位点的股骨颈BMD差异有统计学意义,且CC基因型者的股骨颈BMD低于TT和CT基因型者(CC vs.TT,WMD=-0.025 g/cm^2,P<0.001;CC vs.(CT+TT),WMD=-0.017 g/cm^2,P=0.004);不同XbaI(A/G)位点的股骨颈BMD的差异无统计学意义。经敏感性分析后,发现PvuⅡ(T/C)和XbaI(A/G)位点多态性与腰椎BMD关系的合并结果不稳定。结论在中国成年女性中,Pvu II(T/C)位点多态性与股骨颈BMD有关,且CC纯合子的股骨颈BMD低于T等位基因携带者;尚不能认为XbaI(A/G)位点多态性与股骨颈BMD有关联性。PvuⅡ(T/C)和XbaI(A/G)位点多态性与腰椎BMD的关系有待进一步研究。Objective A meta-analysis was performed to investigate the association between the estrogen receptor α gene (ESR1) PvuII (T/C) and XbaI (A/G) polymorphisms and bone mineral density (BMD) in Chinese adult women. Methods Eligible studies from January 1994 to June 2016 were identified from the following electronic databases: PubMed, OVID MEDLINE, Springer, Web of Science, CBM (Chinese Biomedical Literature Database), CMCI (Chinese Medical Citation Index) and CNKI (Chinese National Knowledge Infrastructure) according to the inclusion and exclusion criteria. Data were extracted and analyzed using Stata 12.0. Results A total of 15 studies with 4753 Chinese adult women were included in this meta-analysis. PvulI (T/C) polymorphism was statistically significantly associated with femoral neck BMD. The mean BMD was significantly lower in the genotype contrasts of CC vs. TT (WMD =-0.025 g/cm2, P〈0.001) and CC vs. (CT+TF) (WMD =- 0.017 g/cm2, P=0.004) in all adult women, while no significant associations between XbaI (A/G) polymorphism and femoral neck BMD were observed for all genotype contrasts (all P〉0.05). Sensitivity analysis results showed that associations of PvuII (T/C) and XbaI (A/G) polymorphisms with lumbar spine BMD were unstable. Conclusion This meta-analysis suggested that in Chinese adult women, there was a strong association between PvuII (T/C) polymorphism and femoral neck BMD and CC homozygotes might have lower BMD when compared with carriers of the T allele, while the XbaI (A/G) polymorphism was not associated with femoral neck BMD. Associations of PvuII (T/C) and XbaI (A/G) polymorphisms with lumbar spine BMD need to be further investigated.
