摘  要:目的探讨过氧化物酶体增殖物激活受体-γ2(PPAR-γ2)基因Pro12Ala多态性与妊娠期糖尿病(GDM)发病的相关性。方法选取400例临床资料完整的产妇,分为GDM组及对照组各200例,对比2组的临床资料,检测并比较2组的PPAR-γ2基因Pro12Ala多态性。结果与正常对照组比较,GDM组年龄较大、孕前BMI较高、孕期增长BMI较多、分娩时孕周较短、经产妇较多、有糖尿病家族史或既往GDM者比例较高(P均<0.05)。GDM组与正常对照组中均能检测出PPAR-γ2基因多态性,基因型为Pro/Pro、Pro/Ala。GDM组Pro/Ala基因型频率为8.0%,Ala等位基因频率为4.0%,与正常对照组中的Ala等位基因频率8.8%比较差异有统计学意义(OR=0.435,95%CI0.236~0.799,P<0.05)。结论 PPAR-γ2基因的等位基因Ala对GDM发病可能具有保护作用。Objective To explore the association between Pro12Ala polymorphism of PPAR-γ2 and the incidence of gestational diabetes mellitus (GDM). Methods In total, 400 pregnant women with complete clinical data were recruited and divided into the GDM (n=200) and control groups (n=200). Clinical data were statistically compared between two groups. The Pro12Ala polymorphism of PPAR-γ2 was detected and compared between the GDM and control groups. Results Compared with the control group, the percentage of individuals with older age, higher prenantal BMI, BMI increase during pregnancy, shorter gestational weeks until delivery, more pluripara women and family history of diabetes mellitus or GDM was significantly higher in the GDM group (all P 〈 0.05). The PPAR-γ2 polymorphism (genotype Pro/Pro and Pro/Ala) were detected in both the GDM and control groups. In the GDM group, the frequency of Pro/Ala was 8.0%. The frequency of Ala allele was 4.0%, significantly lower compared with 8.8% in the control group (OR=0.435, 95%CI 0.236~0.799, P 〈 0.05). Conclusion The Ala allele of PPAR-γ2 probably plays a protective role in the incidence of GDM.
