切开复位内固定术应用于胫腓骨中下段粉碎性骨折的效果探析

摘　　要：目的探析切开复位内固定术应用于胫腓骨中下段粉碎性骨折的效果。方法方便选取该院2012年9月—2015年2月确诊并收治的98例胫腓骨中下段粉碎性骨折患者以就诊序号随机分为研究组和对照组,每组49例。研究组实施切开复位锁定钢板内固定术。对照组实施交锁髓内钉内固定术。对比两组手术时间、术中出血量、完全负重时间、骨折愈合时间、疗效、术后并发症。结果研究组手术时间显著低于对照组(P<0.05),两组术中出血量、正常负重时间及骨折愈合时间比较差异无统计学意义(P>0.05)。研究组优良率95.92%,与对照组91.84%比较差异无统计学意义(P>0.05)。研究组并发症发生率14.29%显著低于对照组48.98%(P<0.05)。结论切开复位内固定术应用于胫腓骨中下段粉碎性骨折与交锁髓内钉内固定术相比疗效及外科创伤相当,均可促进骨折良好恢复,但切开复位锁定板内固定术手术更为便捷,术后并发症少。Objective To discuss the effect of Open reduction and internal fixation in the comminuted fractures of tibiofibulas middle and inferior segment. Methods 98 cases of patients with comminuted fractures of tibiofibulas middle and inferior segment admitted and treated in our hospital from September 2012 to February 2015 were convenient selected and randomly divided into two groups with 49 cases in each, the research group adopted the Open reduction and internal fixation, while the control group adopted the interlocking intramedullary nail fixation, and the operation time, intraoperative bleeding amount, total loading time, fracture healing time, curative effect and postoperative complications were compared between the two groups. Results The operation time in the research group was obviously lower than that in the control group,（P〈0.05）, the differences in the intraoperative bleeding amount, normal loading time and fracture healing time between the two groups had no statistical significance,（P〉0.05）, and the difference in the excellent and good rate between the research group and the control group had statistical significance,（95.92% vs 91.48%）（P〉0.05）, and the incidence rate of complications in the research group was obviously lower than that in the control group,（14.29% vs 48.98%）,（P〈0.05）. Conclusion The curative effect and the surgical wound of pen reduction and internal fixation in the comminuted fractures of tibiofibulas middle and inferior segment is equivalent to that of interlocking intramedullary nail fixation, and both of them can promote the good recovery, but the former is simple with fewer postoperative complications.
