摘 要:目的对比研究动态心电图与常规心电图在诊断冠心病患者心律失常方面的临床选用价值。方法方便选择该院于2014年6月—2015年6月收拾的98例冠心病患者作为研究对象,均先对患者进行常规心电图检查,挑选符合冠心病心律失常的诊断标准的患者。然后用动态心电图24 h持续监测患者的心率变化。最后将检查结果采用统计学软件分析比较两种检查方法对冠心病各种心律失常的检出率有无差异。结果 98例患者进行常规心电图检查诊断阳性52例,阳性率为53.1%;采用动态心电图检查阳性67例,阳性率为68.4%。通过对这两种检测方法检查结果的阳性率对比,发现对于冠心病患者心律失常患者采用常规心电图的检出率明显低于采用动态心电图的检出率,差异有统计学意义(P<0.05);同时动态心电图在室性早搏46例(43.9%)、房性早搏40例(40.8%)、短阵室上速15例(15.3%)等疾病的诊断阳性率亦明显高于常规心电图检出率,差异有统计学意义(P<0.05)。结论动态心电图在监测和诊断冠心病心律失常方面较常规心电图更为灵敏和有效,能够更准确地监测患者的心电信号,它可检测出患者所测时间段内任一时间点的冠脉血流情况和心律失常发作的时间及频率,值得在临床中广泛推广应用。Objective To compare the clinical effect of dynamic electrocardiogram and routine electrocardiogram in diagnosis of myocardial ischemia and arrhythmia of patients with coronary heart disease. Methods Convenient selection 98 cases of patients with coronary heart disease admitted and treated in our hospital from June 2014 to June 2015 were selected as the research objects and all patients adopted the routine electrocardiogram examination, and the patients meeting the coronary heart disease arrhythmia diagnostic standards were selected and then the heart rate changes of patients were monitored around 24 h by the dynamic electrocardiogram, finally, the examination results were statistically analyzed and the difference in the test rate of arrhythmia between the two examination methods was compared. Results In the 98 cases of patients, there were 52 positive patients by the routine electrocardiogram, and the positive rate was 53.1%, and there were 67 positive cases by the dynamic electrocardiogram, and the positive rate was 68.4%, and the test rate of the routine examination was obviously lower than that of the dynamic examination, and the difference had statistical significance(P <0.05), at the same time, there were 46 cases with premature ventricular extrasystole, 40 cases with atrial premature beats and 15 cases with supraventricular tachycardias, and the positive rate was obviously higher than those of the routine examination, and the difference had statistical significance(P<0.05). Conclusion The dynamic electrocardiogram in monitoring and diagnosis of coronary heart disease arrhythmia is more sensitive and effective than the routine electrocardiogram, which can more accurately monitor the electrocardiosignal of patients, and it can detect the coronary blood flow and arrhythmia attack time and frequency at any time point in the detection period, which is worth wide promotion and application in clinic.
