丹参川芎嗪注射剂对骨折术后肢体肿胀患者血清TNF-α、IL-1β及IL-6含量的影响

摘　　要：目的研究丹参川芎嗪注射剂对骨折术后肢体肿胀患者的血清肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)和白细胞介素6(IL-6)水平的影响。方法选择符合纳入标准的骨折术后肢体肿胀患者60例,随机分为甘露醇组、丹参川芎嗪组(简称丹川组)和对照组,各20例。入组患者在围术期均予头孢硫脒预防感染及对症支持治疗。术后当天,甘露醇组予复方甘露醇注射剂250 m L,iv gtt,qd;丹川组予丹参川芎嗪注射剂10 m L,iv gtt,qd;对照组不予以上两种药物。疗程12 d。3组患者分别于入院后第2日(术前)及术后1、3~5、10~12 d清晨空腹采集抽取外周静脉血,应用酶联免疫吸附法(ELISA)测定TNF-α、IL-1β及IL-6在血清中的含量。结果与对照组比较,丹川组于术后3~5 d和术后10~12 d患者血清TNF-α含量均明显降低(P<0.05),甘露醇组于术后10~12 d血清TNF-α和IL-1β含量均降低(P<0.05);与术前比较,丹川组于术后10~12 d血清TNF-α含量明显降低(P<0.05),甘露醇组于术后3~5 d和术后10~12 d血清IL-6含量降低(P<0.05),并于术后10~12 d血清TNF-α、IL-1β含量降低(P<0.05)。结论丹参川芎嗪注射剂能够抑制骨折术后肢体肿胀患者TNF-α的大量释放,对肢体肿胀的减轻有一定的作用。AIM To observe the effect of Danshen Chuanxiongqin injection on the changes of serum con- centrations of tumor necrosis factor alpha （TNF-α） , interleukin-1 （IL-115） and interleukin-6 （IL-6） in patients with limb swelling after fracture surgery. METHODS Totally 60 patients were selected according to the inclusion and exclusion criteria. All the patients were randomly divided into mannitol group, Danshen Chuanxiongqin group and control group, 20 patients in each group. The patients in the mannitol group and Danshen Chuanxiongqin groupwere respectively given compound mannitol injection and Danshen Chuanxiongqin injection on the day of surgery for 12 days, while the control group was not given the above two drugs. Peripheral blood samples were taken for deter- mination of TNF-α, IL-1β and IL-6 in blood serum on the 2nd day after admission and on the 1st, 3rd -5th and 10th - 12th days after operation using ELISA. RESULTS Compared with the control group, TNF-a levels of the Danshen Chuanxiongqin group were much lower at the 3rd -5th and 10th - 12th days after operation（P 〈 0.05 ） , TNF-α and IL-1β levels of the mannitol group were lower at the 10th - 12th days after operation（ P 〈 0.05 ）. Com- pared with the 2nd day after admission, TNF-α levels of the Danshen Chuanxiongqin group decreased significantly at the 10th - 12th days after operation（ P 〈 0.05 ） , IL-6 levels of the mannitol group decreased obviously at the 3th- 5th days and the 10th- 12th days after operation（P 〈0.05）. In addition , TNF-a and IL-1βlevels of the mannitol group also reduced at the 10th -12th days after operation（P 〈 O. 05）. CONCLUSION Danshen Chua- nxiongqin injection may play an important role in inhibiting the release of TNF-α into the blood stream in patients with postoperative limb swelling of fracture and reducing the swelling of the limbs.
