睾丸动脉及精索静脉血流流速与精液质量的关系

摘　　要：目的探讨睾丸动脉及精索静脉血流流速与精液质量的关系。方法选取61例左侧精索静脉曲张(varicocele,VC)患者为VC组,67例无精索静脉曲张的志愿者为对照组,比较两组睾丸动脉血流流速、精索静脉血流流速、阴囊温度及精液质量(A级+B级);给予VC组患者显微镜下精索静脉结扎术,比较术前和术后3个月精液质量和阴囊温度。结果两组睾丸动脉血流流速比较,差异无统计学意义(P>0.05);VC组精索静脉血流速度、精子活力显著低于对照组,睾丸温度高于对照组,差异均有统计学意义(P<0.05)。VC组术后3个月睾丸温度低于术前,精子活力高于术前,差异均有统计学意义(P<0.05)。结论 VC组精索静脉血液回流速度缓慢,可能是引起睾丸温度增高的原因之一,进而造成了精液质量下降,引起男性不育。因此VC患者精索静脉血流速度缓慢是造成男性不育的原因之一。Objective To investigate the relationship of testicular artery and spermatic vein blood flow velocity with the semen quality.Methods Sixty one cases of left varicocele （VC） patients were setted as VC group, and 67 cases of volunteers without varicocele were setted as control group.The testicular artery bloodstream, spermatic vein bloodstream, scrotum temperature and semen quality of two groups were compared.The VC group was given operation of the varicocele under microscope, scrotum temperature and semen quality were compared between preoperative and three months after the operation.Results The testicular artery bloodstream had no statistically significant differences between the two groups （P〉0.05）.The spermatic vein bloodstream of VC group was obviously lower than the control group, the scrotum temperature of was lower than the control group, the semen quality was lower than the control group, and the differences were statistically significant（P〈0.05）.The semen quality of three months after the operation was higher than preoperation, the scrotum temperature was lower than preoperation, and the differences were statistically significant（P〈0.05）.Conclusion The spermatic vein bloodstream of VC patients is slow, the phenomenon may be one of the reasons for testicular temperature increased, thus resulting in the decline of the semen quality and male infertility.The spermatic vein bloodstream of VC patients with slow speed is one of the causes of male infertility.
