摘 要:目的了解影响新生儿血清抗-HBs水平的相关因素和母婴血清抗-HBs之间的关系。方法对120名单胎出生的新生儿及其母亲,分别采集脐带血和静脉血3m L进行抗-HBs定量检测,并对血清抗-HBs滴度进行卡方检验和秩相关分析。结果母亲血清抗-HBs阳性率为58.3%(70/120),新生儿阳性率为60%(72/120),母婴均阳性的67对。新生儿血清抗-HBs水平汉族与回族间(χ~2=0.785,P>0.05)、城乡新生儿间(χ~2=3.261,P>0.05)、不同性别(χ~2=0.731,P>0.05)、不同分娩方式(χ~2=1.806,P>0.05)、不同胎次(χ~2=5.045,P>0.05),差异均无统计学意义。三组不同胎龄的新生儿血清抗-HBs水平差异有统计学意义(χ~2=15.275,P<0.05),随着胎龄的增高,新生儿血清抗-HBs浓度也将增高。对67对母婴血清抗-HBs滴度进行Spearman相关性分析发现,母婴血清抗-HBs滴度呈正相关(rs=0.928,P<0.01)。结论新生儿血清抗-HBs水平不受民族、城乡、性别、分娩方式和胎次的影响,但受胎龄的影响,对于长期没有接种乙肝疫苗的育龄妇女,建议怀孕前进行一针强化免疫。Objective To investigate the relevant factors of neonatal hepatitis B immune level and the relationship between maternal and child hepatitis B antibody levels. Methods Singleton neonates' cord blood and their mother' venous blood were collected and anti -HBs was quantitatively detected. Serum anti -HBs titer was analyzed using Chi square test and rank correlation analysis. Results Mother' anti -HBs positive rate was 58.3% (70/120) while neonate anti -HBs positive rate was 60% (72/120) and 67 pairs were positive at the same time. The anti -HBs level of neonatal serum between Han andHui(χ^2=0.785, P〉0.05 ), between urban and rural areas ( χ^2=3.261 ,P〉0.05 ) and different sex (χ^2=0.731 ,P〉0.05 ), different delivery modes (χ^2=1.806, P〉0.05 ), different parity(χ^2=5.045, P〉0.05 ) showed no significant difference. There was significant difference in anti-HBs levels among three gestational age groups (χ^2=15.275 ,P〈0.05)and with the gestational age raising, the serum anti -HBs titer increasing in Neonates. Using Spearman correlation analysis of 67 maternal and neonatal serum anti -HBs titer, their serum titers of anti-HBs showed a significant positive correlation.Conclusion Neonatal hepatitis B immune level is subjected to gestational age at birth and is not affected by ethnicity,urban-rural areas, gender, mode of delivery and parity. For fertile women without hepatitis B vaccination for a long time, it is necessary to accept enhanced immunity before being pregnancy.
