推广腹膜透析社区化管理模式的效果观察

摘　　要：目的 :探讨推广腹膜透析社区化管理模式的效果。方法 :选取实行腹膜透析社区化管理模式的18例患者作为观察对象,均给予建立病情档案后进行社区化管理干预,由社区卫生服务中心的医护人员对腹透患者进行家庭随访,同时电话随访,定期联系,具体措施包括观察病情、指导患者保持水盐出入平衡、预防感染、定期健康教育等。观察比较社区化管理前后患者的水肿情况、发生感染情况,使用国际协调委员会生活质量评分量表(QOL)衡量干预前后患者的生活质量指数。结果 :推广腹膜透析社区化管理模式3个月后,本组患者重度水肿人数明显少于管理前(P<0.01),外口感染发生率明显低于管理前(P<0.05),患者的QOL评分明显优于管理前(P<0.05)。结论 :推广腹膜透析社区化管理模式对于保障治疗效果、降低感染率、提高患者的生活质量都有明显效果。Objective： To explore the effect of promotion of community management mode of peritoneal dialysis. Methods： Eighteen cases of patients with peritoneal dialysis were selected, to whom the community management mode was implemented. The main intervention measures included the observation of their conditions, guiding patients to keep water-salt balance, infection prevention and regularly health education and so on. The incidence of edema and infection and the indexes of quality of life before and after intervention were compared. Results： After the intervention was lasted for three months, the number of patients with severe edema was less and the incidence of infection was lower and the indexes of quality of life was higher after intervention than before （P〈0.05）. Conclusion： The promotion of community management mode possesses an obvious effect to ensuring the clinical efficacy, reducing the incidence of infection and improving the quality of life.
