老年2型糖尿病合并非酒精性脂肪肝血清nesfatin-1变化及其影响因素

摘　　要：目的探讨老年2型糖尿病(T2DM)合并非酒精性脂肪肝(NAFLD)患者血清nesfatin-1变化及其影响因素。方法选取老年T2DM患者32例(T2DM组)、T2DM合并NAFLD患者32例(T2DM-NAFLD组)及同期健康体检者30例(NC组)。采用ELISA法测定血清nesfatin-1水平,分析其影响因素及其与胰岛素抵抗(IR)的关系。结果 T2DM组和T2DM-NAFLD组患者血清nesfatin-1水平低于NC组,T2DM-NAFLD组患者血清nesfatin-1水平低于T2DM组(P<0.05或P<0.01)。血清nesfatin-1水平与HOMA-IR呈负相关(P<0.05),与HOMA-β呈正相关(P<0.01)。TG、WC是nesfatin-1的独立影响因素(P<0.05或P<0.01)。结论老年T2DM合并NAFLD患者血清nesfatin-1降低可能参与IR及NAFLD的发生。Objective To investigate the change and related factors of serum nesfatin-1 level in elderly patients with type 2 diabetes mellitus （T2DM） and non-alcoholic fatty liver disease （NAFLD） . Method Serum nesfatin-1 concentration was examined in 32 T2DM patients without NAFLD （ T2DM group） , 32 T2DM patients with NAFLD （T2DM-NAFLD group） and 3 0 cases of normal control （NC group） .The influencing factors for nesfatin-1 were analyzed. Results Compared with the other two groups, serum nesfatin-1 concentration was the lowest in T2DM- NAFLD group （P〈0.05 or P〈0.01） . TG and WC were the independent related factors for nesfatin-1 （P〈0.05 or P〈0.01） Conclusion Serum nesfatin-1 concentration obviously declined in elderly T2DM patients with NAFLD, which may be involved in the development of IR and NAFLD.
