中西医结合治疗小儿呼吸机相关性肺炎疗效分析

摘　　要：目的:评价中西医结合治疗小儿呼吸机相关性肺炎(VAP)的临床疗效。方法:选取小儿VAP患者160例,随机分为对照组和治疗组,每组各80例。对照组患者给予西医常规治疗,治疗组患者予中药方剂雾化,辅以脐部贴敷、耳穴贴压和中药漱口液治疗,观察2组患者临床疗效、中医证候疗效、血清降钙素原、肿瘤坏死因子水平、C-反应蛋白水平、械通气时间和脱机成功率,评价中西医结合疗法的临床疗效。结果:治疗后患者临床总有效率为87.5%,显著高于常规治疗组(67.5%),治疗组中医证候疗效、血清降钙素原、肿瘤坏死因子水平、C-反应蛋白水平、械通气时间和脱机成功率均显著优于对照组(P<0.05)。结论:中西医结合疗法在缓解小儿VAP症状的同时,无明显毒副作用,提高了患者的治愈率,治疗方法安全可靠,疗效确切。Objective ： to evaluate the curative effect of integrated traditional Chinese and western medicine in treatment of ventila-tor associated pneumonia （VAP） . Methods ： 160 VAP children were randomly divided into control group and treatment group. The former were treated with western conventional therapy and the latter with Chinese medicinal atomization, combining with application therapy to unbilical region, auricular point sticking and Chinese medicinal collutory. Observe the clinical effects, effects of TCM Syn-drome ,serum procalcitonin, TNF （tomor necrosis factor） level, CRP （ C-reaction protein） level, duration of mechanical ventilation and weaning success rate, evaluating the clinical effect of the integrated therapy. Results： After treatment, the total effective rate was 87. 5% , significantly higher than that of control group （67. 5% ） , effect of TCM Syndrome, procalcitonin, TNF, CRP, duration of me-chanical ventilation and weaning success rate were all superior to that of control group （ P〈0. 05） . Conclusion： The integrated ther-apy, safe and reliable, can relieve symptoms of VAP without obvious toxic and side effect, and improve the recovery rate.
