继发型肺结核患者下呼吸道内赛多孢子菌定植初探

摘　　要：目的初步了解赛多孢子菌(Scedosporium)在继发型肺结核患者下呼吸道内感染或定植状况。方法收集101例继发型肺结核患者的支气管肺泡灌洗液,分别使用真菌镜检、选择培养基培养及扩增部分β-微管蛋白基因的聚合酶链式反应-反转线状点杂交(PCR-RLB)方法,检测常见的5种赛多孢子菌病的致病菌,即S.apiospermum、S.boydii、S.minutispora、S.aurantiacum及S.dehoogii。结果 101份受检标本均未检测到上述5种赛多孢子菌。结论在继发型肺结核患者中未见常见5种赛多孢子菌呼吸道感染或定植,提示继发型肺结核患者并发赛多孢子菌病的可能性不大。Objective Understanding colonization or infection of Scedosporiumin lower respiratory tract of secondary pulmonary tuberculosis.Methods Major pathogens of Scedosporiosis,5 Scedosporium spp.（S.apiospermum,S.boydii,S.minutispora,S.aurantiacumand S.dehoogii）,were detected in bronchoalveolar lavage fluid from the patients with secondary pulmonary tuberculosis using direct microscope examination,fungal culturing and PCR-based reverse line blot（PCR-RLB）targeting the partialβ-tubulin gene.Result No Scedosporiumspp.were found in 101 specimens of bronchoalveolar lavage fluid from secondary pulmonary tuberculosis patients.Conclusion Rare Scedosporiumspp.colonize or infect in lower respiratory tract of secondary pulmonary tuberculosis.It implies that secondary pulmonary tuberculosis complicated by Scedosporiosis due to increasing colonization of Scedosporium spp.intherespiratory tract will be uncommon.
