综合关怀改善血友病患者心理健康状况的效果分析

摘　　要：目的研究综合关怀对改善血友病患者心理健康状况的效果。方法在2014年8月采用焦虑自评量表(self-rating anxiety scale,SAS)、抑郁自评量表(self-rating depression scale,SDS)和心理症状自评量表(self-reporting inventory,SCL-90)测量泰安市某三甲医院登记注册的血友病患者综合关怀前后的心理健康状况变化,包括SAS、SDS、SCL-90总分以及躯体化、强迫症状、人际关系敏感、抑郁、焦虑、敌对、恐怖、偏执、精神病性的因子得分,并将测试得分与国内常模进行对比分析。计量资料采用t检验,P<0.05为有统计学意义。结果实施综合关怀后患者的SAS、SDS和SCL-90总分及各因子测试得分[(44.95±10.88)、(48.17±13.67)、(159.91±48.75)、(1.59±0.51)、(2.04±0.65)、(1.88±0.64)、(1.89±0.72)、(1.70±0.61)、(1.77±0.69)、(1.51±0.58)、(1.70±0.66)、(1.70±0.57)分]与综合关怀前测试得分[(5 4.8 4±10.49)、(53.70±12.34)、(180.63±45.47)、(1.93±0.44)、(2.14±0.63)、(2.01±0.65)、(2.02±0.75)、(2.03±0.50)、(2.02±0.61)、(1.92±0.46)、(1.95±0.59)、(1.91±0.54)分]相比明显下降,但干预前后得分均明显高于国内常模[(41.88±10.57)、(33.80±5.90)、(129.96±38.76)、(1.37±0.48)、(1.62±0.58)、(1.65±0.51)、(1.50±0.59)、(1.39±0.43)、(1.48±0.56)、(1.23±0.41)、(1.43±0.57)、(1.29±0.42)分],对比差异均有统计学意义(均P<0.05)。结论血友病患者的心理健康状况不佳,综合关怀能够显著提高患者心理健康水平,但关怀工作应根据效果评价持续推行,不断提高血友病患者的生存质量。Objective To study the effection of hemophilia comprehensive care to the mental health status of the hemophilia patients.Methods The mental health status of registered hemophiliac of a third-grade class-A hospital in Taian were measured by the common self-rating anxiety scale（SAS）,self-rating depression scale（SDS）,psychological symptom checklist（SCL-90） and each score of somatization,obsessive-compulsive symptoms,interpersonal sensitivity,depression,a nxiety,hostility,horror, paranoid and psychosis in August 2014.The changes of mental health status of hemophilia patients from post-intervention to pre-intervention were assessed.And these changes were compared with the national norm. Measurement data was processed by t test,the result of P〈0.05 was considered statistically significant.Results After hemophilia comprehensive care,the SAS,SDA and SCL-90 [（44.95±10.88）,（48.17±13.67）,（159.91±48.75）, （1.59±0.51）,（2.04 ±0.65）,（1.88±0.64）,（1.89±0.72）,（1.70±0.61）,（1.77±0.69）, （1.51±0.58）, （1.70±0.66）,（1.70±0.57） scores] of 54 hemophiliac who were after hemophilia comprehensive care were significantly lower than that of 54 hemophiliac who were before hemophilia comprehensive care[（54.84±10.49）,（53.70±12.34）,（180.63±45.47）,（1.93±0.44）, （2.14±0.63）,（2.01±0.65）,（2.02±0.75）, （2.03±0.50）,（2.02±0.61）,（1.92±0.46）,（1.95±0.59）,（1.91±0.54）scores],while all the corresponding scores were significantly higher than the national norm [（41.88±10.57）,（33.80±5.90）,（129.96±38 .76）,（1.37±0.48）,（1.62±0.58）,（1.65±0.51）,（1.50±0.59）,（1.39 ±0.43）,（1.48±0.56）,（1.23±0.41）,（1.43±0.57）,（1.29±0.42） scores]（all P〈0.05）.Conelusion Mental health of people with hemophilia is not optimistic.Comprehensive care of hemophilia can significantly improve mental health level of patients,but the comprehensive care work should be done continuously based on performance eva
