摘 要:目的探讨红细胞体积分布宽度(RDW)、超敏C-反应蛋白(hs-CRP)、Apgar评分在新生儿重症监护病房(ICU)中的应用价值。方法选取2015年1月至2016年4月收治的94例新生儿ICU患者,根据病情变化情况将其分为好转组(n=33)、无变化组(n=31)与恶化组(n=30),另选取同期30例非ICU新生儿作为对照组。依据Apgar评分量表对研究对象健康状况进行评估,采集新生儿静脉血3 ml,以SIMENS BNpro Spec全自动特定蛋白分析仪检测hs-CRP表达水平、以Sysmex XE-5000全自动血液分析仪检测RDW。结果好转组、无变化组Apgar评分低于对照组,住院天数多于对照组,差异有统计学意义(P<0.05),RDW、hs-CRP比较差异未见统计学意义(P>0.05);无变化组RDW高于好转组,差异有统计学意义(P<0.05),两组其余各指标比较差异未见统计学意义(P>0.05);恶化组Apgar评分低于对照组,RDW、hs-CRP、住院天数高于对照组,差异有统计学意义(P<0.05);hs-CRP与RDW呈正相关(r=0.354,P<0.05)、Apgar评分与RDW呈负相(r=-0.411,P<0.05);RDW与住院天数间呈正相关(r=0.732,P<0.05)、Apgar评分与住院天数间呈负相关(r=-0.572,P<0.05)。结论 RDW在新生儿ICU病情转归及预后评估中具有重要临床价值,联合hs-CRP与Apgar评分可更有效评定新生儿病情变化情况、预测住院时间。Objective To investigate the application value of red blood cell volume distribution width (RDW), high sensitive C-reactiveprotein (hs-CRP) , Apgar score in the neonatal intensive care unit (ICU). Methods Ninety-four casesof neonatal ICU from January 2015 to April 2016 were selected, according to the change of condition, they were dividedinto improved group ( n = 33 ) , no change group ( n = 31 ) and deterioration group ( n = 30) , another 30 cases who were non- ICU in the same period were selected as control group. According to the Apgar score scale, the health status of the research objects, collectingneouatal venous blood 3 ml, the expression level of hs-CRP was detected by SIMENS to BNproSpccautomatic spccificprotein analyzer, with Sysmex XE-5000 automatic blood analyzer for RDW. Results The Ap- gar score of the improved group and no change group was lower than that of the control group, the number of days in hospitalwas more than that in the control group, the difference was significant ( P 〈 0. 05 ) , and there was no significant difference in hs-CRP or RDW ( P 〉 0. 05 ). RDW in no change group was higher than that in the improved group, the difference was significant ( P 〈 0. 05 ) , the other indicators between the two groups had no significant difference ( P 〉 0. 05). The Apgar score in deterioration group was lower than that in the control group, hs-CRP, RDW, the number of days in hospital were higher than those in the control group, the differences were significant ( P 〈 0. 05 ) ; hs-CRPwas posi- tively correlated with RDW ( r = 0. 354, P 〈 0. 05 ) , and Apgar score was negatively correlated with RDW ( r = - 0. 411, P 〈 0. 05 ). There was positive correlation of RDW with the length of stay ( r = 0. 732, P 〈 0. 05 ) , and negative correlation of the Apgar score with the length of stay ( r = - 0. 572,P 〈 0. 05 ).
