摘  要:目的观察核因子相关因子2(nuclearfactor—erytkrorid2-relatedfactor,Nrt2)及血红素氧合酶.1(HO-1)在兔海水及淡水淹溺型急性肺损伤中的表达。方法18只新西兰兔按数字法随机分为海水组(经气管内注射3ml/kg配方海水,n=6)、淡水组(经气管内注射18ml/kg淡水,n=6)和对照组(n=6)。模型建立成功后于0、0.5、1、3及6h抽取动脉血行血气分析。模型建立成功后6h处死兔,留取肺组织标本。检测兔肺湿干质量比值,肺通透指数,支气管肺泡灌洗液中TNF-α、IL-1β、IL-6和IL-10水平,观察兔肺组织病理变化。检测兔肺组织中MDA含量、SOD水平和caspase-3蛋白水平。检测兔肺组织中Nrt2、HO.1mRNA及蛋白水平。结果海水组和淡水组兔肺湿干质量比值和肺通透指数较对照组明显增加(P〈0.05)。海水组和淡水组兔支气管肺泡灌洗液中促炎细胞因子和抑炎细胞因子水平均较对照组明显增高(P〈0.05)。海水组和淡水组兔TNF-01分别为(121.8±22.7)mg/L和(113.5±23.0)mg/L,对照组为(82.8±16.5)mg/L。海水组和淡水组IL-1β分别为(76.4±15.5)mg/L和(62.2±12.8)mg/L,对照组为(32.24-7.4)mg/L。海水组和淡水组IL-6分别为(34.1±7.6)mg/L和(35.0±7.4)mg/L,对照组为(16.5±3.8)mg/L。海水组和淡水组IL-10分别为(22.1±3.9)mg/L和(19.0±2.8)mg/L,对照组为(13.4±2.8)mg/L。海水组和淡水组肺损伤较对照组明显加重,海水组和淡水组肺组织中MDA含量及SOD水平较对照组升高(P〈0.05)。海水组和淡水组肺组织中caspase-3蛋白水平较对照组升高(P〈0.05)。海水组和淡水组肺组织中Nrf2及HO-1mRNA及蛋白水平较对照组升高(P〈0.05)。海水组与淡水组之间各项指标差异均无统计学意义(P〉0.05)。结论海水淹溺型肺损伤与淡水淹溺型肺�Objective To observe the expression levels of nuclear factor-erythroid 2-related factor (Nrf2) and heme oxygenase-1 (HO-1) in acute pulmonary injury induced by seawater and freshwater drowning. Methods Eighteen New Zealand rabbits were randomly assigned to the 3 groups: the seawater group, the freshwater group and the control group, each consisting of 6 animals. In the seawater group, seawater was injected into the trachea at a dosage of 3mg/kg, while in the freshwater group, freshwater was injected into the trachea at a dosage oflSml/kg. Upon establishment of the animal model, arterial oxygen partial pressure levels were detected at Oh, 0.5h, lh, 3h and 6h, after establishment of the model. The animals were sacrificed and lung tissue samples were taken for the detection of lung wet/dry weight (W/D) ratio, pulmonary permeability data and tumor necrosis faetor-α ( TNF-α ), interleukin-1β ( IL-1β), IL-6 and IL-10 levels in bronchoalveolar lavage fluid (BALF) . At the same time, changes in the histopathology of the pulmonary tissue were also detected. Lung specimens were evaluated by hematoxylin-eosin staining. Malondialdehyde (MDA) and superoxide dismutase (SOD) levels were also assayed, and caspase-3, Nrf2 and HO-1 expression levels were measured by quantitative real-time PCR and Western blot. Results Oxygenation levels of the seawater and freshwater groups Were dramatically decreased, as compared with those of the control group ( P 〈 0.05 ). The W/D ratio and pulmonary permeability data of the seawater and freshwater groups were significantly increased, as compared with those of the control group. Furthermore, the values of TNF-α, IL-6, IL-1 β and IL-10 were also significantly increased in BALF as compared with those of the control group( P 〈0.05 ). TNF- ot values of the seawater and freshwater groups were respectively (121.8 ± 22.7) pg/ml and (113.5 ± 23.0) pg/ml, while that of the control group was (82.8 ± 16.5) pg/ml.
