后路椎弓根螺钉固定及经椎弓根植骨治疗胸腰段脊柱爆裂性骨折临床效果

摘　　要：目的:探讨后路椎弓根螺钉固定及经椎弓根植骨治疗胸腰段脊柱爆裂性骨折临床效果。方法:随机纳入2013年10月-2014年10月笔者所在医院骨伤科收治的33例胸腰段脊柱爆裂性骨折患者,采用后路椎弓根螺钉固定及经椎弓根植骨治疗方案,对比治疗前后入组病例神经功能恢复、VAS评分和伤椎椎体指标。结果:随访观察发现,本组患者术后神经功能和VAS评分有大幅改善,无内固定松动和断钉并发症;术后2年椎体高度基本恢复至正常数值,胸腰椎Cobb角为(5.1±1.6)°,与手术前相比差异有统计学意义(P<0.05)。结论:后路椎弓根螺钉固定及经椎弓根植骨治疗胸腰段脊柱爆裂性骨折效果满意,可实现脊柱稳定性的早期重建,避免晚期椎体高度和矫正角度再丢失,值得予以推广。Objective：To investigate the effect of posterior pedicle screw fixation and transpedicular bone grafting in the treatment of thoracolumbar burst fracture clinical effect.Method：From October 2013 to October 2014,33 cases of thoracolumbar burst fractures in our hospital were selected for the study,received the posterior pedicle screw fixation and pedicle bone graft treatment.Neurological function recovery,VAS score and the index of injury and cone of the patients before and after treatment were compared.Result：The follow-up found,this group of patients with postoperative neurological function and VAS score improved significantly,no internal fixation loosening and breakage of complications;after 2 years,the height of vertebral body basically recovered to the normal value,and the Cobb angle of the thoracolumbar spine was（5.1±1.6）°,it was significance better than before treatment（P0.05）.Conclusion：Posterior pedicle screw fixation and transpedicular bone grafting for the treatment of thoracolumbar burst fractures with satisfactory effect,can realize the early reconstruction of spinal stability,and avoid angle correction of late loss of vertebral height,is worthy of promotion.
