摘  要:目的:调查笔者所在医院产科病房新生儿皮肤感染及接触新生儿的医护人员带菌情况,探讨降低母婴同室新生儿皮肤感染的措施。方法:前瞻性研究2015年1-12月笔者所在医院产科病房新生儿皮肤感染与接触新生儿医护人员的带菌情况。对新生儿皮肤感染取样后进行细菌培养及菌株鉴定。每周定期对医护人员手及鼻前庭进行取样,标本进行细菌分离和鉴定。结果:2015年笔者所在医院产科病房新生儿4032例,发生皮肤感染78例,皮肤感染的发生率为1.94%,上半年与下半年比较差异无统计学意义(P>0.05)。共检出病原菌52株,金黄色葡萄球菌(金葡菌)最常见,占25.0%。接触新生儿医护人员手及鼻腔样本共978例,细菌检测阳性率为19.3%。上半年及下半年细菌检测阳性率分别为24.1%及14.5%,两者之间差异有统计学意义(P<0.05)。鼻腔样本细菌检测阳性率为26.4%,手样本细菌检测阳性率为12.3%,鼻腔样本细菌检测阳性率明显高于手样本(P<0.05)。结论:金黄色葡萄球菌是新生儿皮肤感染主要的致病菌。加强医护人员手及鼻腔卫生的培训及监管,可有效降低带菌率。Objective:To investigate of the staff carrying germ and neonatal skin infection in maternal-neonatal unit so as to explore the measures to reduce neonatal skin infection in maternal-neonatal unit.Method:A prospective study on the neonatal skin infection and the staff carrying germ were evolved from January 2015 to December 2015.The bacterial culture and strain identification were carried out after the sampling of neonatal skin infection,and the hand and nasal vestibule of the medical staff were sampled weekly.Result:In 2015,there were 4032 cases in maternal-neonatal unit.The neonatal skin infection rate was 1.94%.There was no difference between the first half and the second half(P0.05).Totally,52 pathogens were found.The major pathogens was Staphylococcus aureus.The results showed that 978 cases were detected in the hand and nasal vestibule of the medical staff.The carrier rates was 19.3%.The carrier rates of medical staff hands and nasal cavity in the second half were significantly lower than the first half(P0.05).The carrier rates of medical staff hands and nasal cavity were 12.3% and 26.4% respectively.The carrier rates in nasal samples were significantly higher than the hands(P0.05).Conclusion:The major pathogens of neonatal skin infection in maternal-neonatal unit is Staphylococcus aureus.The carrier rates can be reduced by strengthening the education to the medical staff on hands and nasal health.
