自体富血小板凝胶联合负压引流治疗老年糖尿病足的临床探讨

摘　　要：目的探讨自体富血小板凝胶联合负压引流治疗老年糖尿病足的临床效果。方法随机选取2012年7月—2016年8月该院治疗的老年糖尿病足60例,按入院顺序编号1~60,并按奇偶数分组,奇数组为观察组,采用自体富血小板凝胶联合负压引流治疗,偶数组为对照组,采用常规外科治疗方法,观察治疗效果并进行安全性评价。结果观察组愈合80.0%,对照组愈合40.0%,对照组平均愈合时间为(15.2±4.1)d,观察组为(10.4±1.2)d,组间比较(P<0.05)。对照组4例发生创口感染,不良反应发生率为26.7%,观察组无不良反应,组间比较,差异有统计学意义(P<0.05)。结论针对老年糖尿病足患者采用自体富血小板凝胶联合负压引流治疗,能够快速控制感染,促进溃疡愈合,极大的缩短伤口愈合时间,且应用安全,不仅为患者解除了痛苦,也缓解了患者的经济负担,是一种临床值得推广的方法。Objective To discuss the clinical effect of autologous platelet-rich gel and negative pressure drainage in treatment of senile diabetic foot.Methods 60 cases of senile patients with diabetic foot treated in our hospital from July 2012 to August 2016 were random selected and divided into two groups according to the admission order,the observation group adopted the autologous platelet-rich gel and negative pressure drainage treatment,while the control group adopted the routine surgical treatment method,and the treatment effect was observed and the safety was evaluated.Results The differences in the healing rate,average healing time an incidence rate of adverse reactions between the observation group and the control group had statistical significance（P〈0.05）[80.0%,（15.2±4.1）d,26.7% vs 40.0%,（10.4±1.2）d,0.0%],and in the control group,there were 4 cases with wound infection.Conclusion The autologous platelet-rich gel and negative pressure drainage in treatment of senile diabetic foot can rapidly control infection,promote the ulcer healing,greatly shorten the wound healing time,and the application is safe,which can not only help patients relieve pains,but also relieve the economic burden of patients,and it is worth clinical promotion.
