化疗相关性血象下降的中医药干预与治疗应用研究

摘　　要：目的观察应用中药组方对恶性肿瘤患者化疗相关性血象下降的临床干预效果,并探讨其可能的作用机制。方法将150例行化疗的恶性肿瘤患者随机分为2组。对照组75例,予鲨肝醇片干预治疗;治疗组75例,于化疗第5 d后给予补肾益髓中药干预治疗。干预治疗21 d后,观察比较2组治疗后生活质量、体力情况、血常规指标[白细胞计数(WBC)、红细胞计数(RBC)、血红蛋白(Hgb)及血小板计数(PLT)]、免疫学指标(包括T淋巴细胞亚群CD3^+、CD4^+、CD8^+及CD4^+/CD8^+)及中医证候的变化。结果治疗组治疗后生活质量改善显效率76.0%,体力恢复改善率73.3%,对照组治疗后生活质量改善显效率48.0%,体力恢复改善率50.7%,治疗组生活质量改善情况及体力恢复情况均优于对照组(P<0.05);2组治疗前血常规WBC、RBC、Hgb及PLT水平组间比较差异均无统计学意义(P>0.05),但治疗组治疗后WBC、RBC、Hgb及PLT水平均高于对照组,比较差异均有统计学意义(P<0.05);2组治疗前免疫学指标T淋巴细胞亚群CD3^+、CD4^+、CD8^+及CD4^+/CD8^+水平组间比较差异均无统计学意义(P>0.05),但治疗组治疗后T淋巴细胞CD3^+、CD4^+及CD4^+/CD8^+水平高于对照组,CD8^+水平低于对照组,比较差异均有统计学意义(P<0.05);2组治疗前各中医证候情况组间比较差异均无统计学意义(P>0.05),2组治疗后各中医证候情况组间比较差异仍均无统计学意义(P>0.05)。结论补肾益髓中药可有效改善恶性肿瘤患者化疗后血象下降,升高WBC、RBC、Hgb及PLT水平,促进体力恢复,提高生活质量,其作用机制可能与保护骨髓造血功能,增强患者免疫功能有关。Objective To observe the clinical intervention effects of traaitiooal Chinese medicine （ TCM）prescription on chemotherapy - related hemogram reduction in patients with malignant tumor,and explore its possible mechanism. Methods 150 patients with malignant tumors were randomly divided into two groups. The control group （n = 75） was treated by Batilol tablets. The treatment group （ n = 75） was treated by Bushen - yisui Chinese medicine after the fifth day of chemotherapy. The quality of life, physical condition, routine blood index, immunolog-ical indicators and TCM syndromes ater 21 d intervention were observed in two groups. The rocludes white blood cell count（WBC）,red blood cell count （RBC）,hemoglobin （Hgb） and platelet count （ PLT）,and immunological indicators includes T lymphocyte subsets CD3^＋ , CD4＋,CD8^＋ and CD4＋/CD8^＋ . Results The improvement rate of quality of life and the physical recovery rate in treatmentspectively ,and in control group was 48.0% and 50.7% , respectively. The improvements in treatment group was su-perior to those in control group （P 〈0. 05）. There was no statistical difference on WBC,RBC, Hgb and PLT before treatment between two groups （ P 〉 0. 05 ）. The WBC RBC,Hgb and PLT after treatment in treatment group were higher than those in control group （ P 〈 0. 05 ）. There was no statistical difference on CD3^＋, CD4^＋, CD8^＋ and CD4^＋ /CD8＋ before treatment between two groups （P 〉0.05）. The CD3＋,CD4＋,and CD4＋/CD8＋ after treat-ment in treatment group were higher than those in control group （ P〈0. 05 ）,and the CD8^＋ was lower （ P〈0. 05 ） .There was no statistical difference on TCM syndromes between before and ater treatment in two groups （ P〉0. 05 ） .Conclusion The Bushen -yisui Chinese medicine can improve the blood flow ater chemotherapy , increase the level of WBC , RBC, Hgb and PLT, promote physical recovery and enhance the quality of life,
