卡介菌多糖核酸联合紫外线光疗对银屑病患者细胞免疫功能影响

摘　　要：目的探讨卡介菌多糖核酸联合紫外线(UVB)光疗对银屑病患者的临床疗效及对细胞免疫功能的影响。方法选取90例寻常型银屑病患者,随机分为对照1组、对照2组和治疗组,每组30例分别给予UVB光疗法、卡介菌多糖核酸、卡介菌多糖核酸联合UVB光疗法进行治疗,比较3组临床疗效及对患者免疫功能的影响。结果治疗组总有效率(93.33%)显著高于对照1组(70.00%)和对照2组(66.66%)(P<0.05)。与治疗前比较,3组患者CD3^+、CD4^+及CD4^+/CD8^+比值显著升高(P<0.05),CD8^+显著降低(P<.05),治疗组变化更为明显(P<0.05)。与治疗前比较,治疗后3组患者各时间段PASI评分均显著下降(P<0.05);治疗组治疗后各时间段PASI评分下降更明显(P<0.05)。治疗组复发率显著低于对照1组和对照2组(P<0.05)。结论卡介菌多糖核酸联合UVB光疗可显著减少皮损面积、降低严重性,有效改善患者细胞免疫功能,是一种可靠的治疗寻常型银屑病的方法。Objective To investigate the cellular immune function in patients with psoriasis receiving BCG polysaccharide and nucleic acid combine with UVB phototherapy.Methods Ninety patients with psoriasis were randomly assigned in three groups：30 patients received UVB phototherapy（control group 1）,30 patients received BCG polysaccharide and nucleic acid（control group 2） and 30 patients received BCG polysaccharide and nucleic acid combine with UVB phototherapy（study group）.The therapeutic effect and cellular immune function of three groups were compared.Results The effective rate of study group（93.33%） were higher than those in control group 1（70.00%） and control group 2（66.66%）（P 〈 0.05）.The CD3~＋,CD4~＋ and CD4~＋/CD8~＋ levels were significantly increase（P〈 0.05）,and the CD8＋ level was significantly decreased after treatment in all three groups（P〈0.05）,while those indicators in study group were improved more significantly than other two groups（P〈0.05）.The PASI scores in three groups were significantly lower than that before treatment（P〈0.05）,while the score in study group was improved more significantly than two control groups（P〈0.05）.The recurrence rate of study group was lower than that in two control groups（P〈0.05）.Conclusion BCG polysaccharide and nucleic acid combine with UVB phototherapy can reduce psoriasis area and severity,improve the cellular immune function in patients with psoriasis.
