开颅微血管减压治疗老年三叉神经痛患者策略及疗效研究

摘　　要：目的探讨微血管减压手术治疗老年三叉神经痛患者的策略及临床疗效。方法回顾性分析2008年1月至2012年12月1100例行三叉神经微血管减压手术患者的临床资料，其中≥65岁患者285例为老年组，〈65岁患者815例为非老年组，出现复发或随访期结束视为终点。比较两组疗效差异。结果老年组患者手术疗效与非老年组比较差异无统计学意义，各项并发症两组差异无统计学意义，但并发症累计发生率两组差异有统计学意义。结论通过严谨的围手术期处理，对能够耐受全身麻醉手术的体健老年三叉神经痛患者，开颅微血管减压手术作为唯一针对病因的治疗手段，仍可作为首选的治疗方式。Objective To investigate the clinical outcome of microvascular decompression for elderly patients with trigeminal neuralgia by using a retrospective study and discuss the technical note. Methods Patients with trigeminal neuralgia hospitalized at Hangzhou First People＇ s Hospital between 2008 and 2012 were chosen for a retrospective study.The patients were divided into elderly group （ 285cases ） and younger group （ 815 cases ） . The clinical efficacy was followed for 42 months after surgery.Chi-square test for count data, t test for measurement data, and the calculation of survival rate was by life-table method and Gehan score test. Results No statistically marked differences were observed between the 2 groups in terms of rate of pain control and each complication.But the total complications were statistically different. Conclusions MVD for healthy elderly patients with TN can still be recommended as a primary method with careful perioperative management.
