神经节苷脂GM1治疗新生儿缺氧缺血性脑病50例临床观察

摘　　要：目的探讨神经节苷脂GM1治疗新生儿缺氧缺血性脑病(hypoxic-ischem icencephalopathy,HIE)的预后及相关炎症指标变化。方法选择2014年6月—2016年6月收治的98例HIE患儿,随机分为观察组50例和对照组48例,对照组在综合治疗基础上给予胞二磷胆碱治疗,观察组在综合治疗基础上给予神经节苷脂GM1治疗,比较两组治疗效果,采用SPSS20.0进行统计学分析,P<0.05为差异有统计学意义。结果观察组总有效率为94.00%,对照组为79.17%,两组比较差异有统计学意义(χ~2=4.683,P<0.05);观察组治疗后反射恢复时间、肌张力恢复时间、意识恢复时间均低于对照组,差异均有统计学意义(均P<0.05);观察组治疗后新生儿行为神经(neonatal behavioral neurological,NBNA)评分为(36.32±4.17)分,高于对照组的(34.79±3.50)分,两组比较差异有统计学意义(t=7.096,P<0.05)。结论神经节苷脂GM1治疗HIE疗效显著,可有效改善患儿神经功能和炎症状态,促进反射、肌张力和意识恢复,可作为优选治疗药物。Objective To observe the efficacy of the ganglioside GM1 therapeutic neonatal hypoxic-ischemic encephalopathy （HIE）. Methods 98 cases with HIE from June 2014 to June 2016 were selected and randomly divided into the observation group （50 cases） and the control group （48 cases）, the control group was given CDPC based on comprehensive treatment, the observation group was given Ganglioside GM1 based on comprehensive treatment. The treatment effect of the two groups was compared. The data was statistically analyzed by SPSS20.0, if P〈0.05, difference had statistical sense. Results The total effective rate of the observation group and the control group was 79.17%, 94.00%, respectively. Treatment effect and total efficiency of the observation group were higher than the control group, the difference was statistically significant （P〈0.05）; reflex time, muscle tone recovery recovery time of the observation group after treatment were lower than the control group, the difference was statistically significant （P〈0.05）; NBNA of the observation group （36.32 ± 4.17） after treatment was higher than the control group （34.79±3.50）, the difference was statistically significant （P〈O.05）. Conclusion Ganglioside GM1 has a significant therapeutic effect in the treatment of HIE, and it can improve neurological function and inflammatory state, promote reflect, muscular tone and consciousness recovery.
