摘 要:目的调查研究一起羊养殖场中发生的人畜共患羊种布鲁氏菌病(布病)暴发疫情的流行病学和分子特征。方法血清学实验采用试管凝集试验检测布病抗体,采用纸片法检测菌株的药物敏感性,采用多位点可变数目串联重复序列分析(MLVA)和多位点序列分析(MLST)分析菌株的分子特征。结果工作人员布鲁氏菌抗体阳性率为7/18,饲养羊的阳性率为5/8。2名工作人员血液分离培养出羊种布鲁氏菌(菌株编号:CQ-18和CQ-11),3只羊的血液分离培养出羊种布鲁氏菌(菌株编号:CQ-154、CQ-159和CQ-237)。药敏实验结果:5株菌对环丙沙星、左氧氟沙星、强力霉素、头孢曲松、诺氟沙星、米诺霉素、莫西沙星、氧氟沙星、利福平和链霉素敏感,CQ-154和CQ-237菌株对复方新诺明敏感,CQ-159、CQ-18和CQ-11菌株对复方新诺明耐药,5株菌均对阿奇霉素和克拉霉素耐药。MLVA分析发现,CQ-154菌株在BRU16位点少了一个串联重复序列,菌株CQ-18在BRU04位点增加了一个串联重复序列,MLST分析5株菌株ST型为8型。结论布鲁氏菌长期在动物体内存在,是人畜感染布病无法根治的一个关键问题。在我国首次发现对复方新诺明耐药的羊种布鲁氏菌菌株。Objective To study on epidemiology and molecular characteristics of Brucella melitensis during an zoonotic disease outbreak of brucellosis in sheep farm. Methods Brucella antibodies were detected by serum agglutination testing( SAT). The disc diffusion method was used for antibiotic susceptibility testing of Brucella melitensis strains. Molecular characteristics was analyzed by multi-variable-number tandem repeat analysis( MLVA) and multilocus sequence typing( MLST). Results The seroprevalence of Brucella antibodies in humans and sheep in sheep farm was 7/18 and 5/8respectively. Two Brucella melitensis strains were cultured from the blood of two sheep keepers( No. CQ-18 and CQ-11).Three Brucella melitensis strains were cultured from the blood of sheep( No. CQ-154,CQ-159 and CQ-237). Antibiotic susceptibility test results showed that five strains were sensitive to ciprofloxacin,levofloxacin,doxycycline,ceftriaxone,norfloxacin,minocycline,moxifloxacin,ofloxacin,rifampicin and streptomycin. Two strains of CQ-154 and CQ-237 were sensitive to SMZ-TMP. Three strains of CQ-159,CQ-18 and CQ-11 were resistant to SMZ-TMP. All five strains were resistant to azithromycin and clarithromycin. MLVA analysis found that the strain CQ-154 lacked a tandem repeat in the BRU16 locus. The strain CQ-18 had an increased tandem repeat in the BRU04 locus. MLST results showed that all isolated strains could be classified into a 8 ST type. Conclusion It was a key problem for people and livestock can not cure the infection of brucellosis because of Brucella can harbor within animal body for a long time. It was the first time to find that Brucella melitensis was resistant to SMZ-TMP in China.
