真皮脂肪瓣联合“┼”字缝线矫正重度乳头内陷的效果

摘　　要：目的探讨真皮脂肪瓣联合"┼"字缝线支撑矫正重度乳头内陷的临床效果。方法选取2014年6月至2016年4月于安徽省立医院整形外科就诊的15例(29侧)重度乳头内陷患者,采用真皮脂肪瓣交叉支撑乳头基底,同时瓣下以缝线做"┼"字交叉支撑外接牵引器悬吊4周,术后随访6个月~1年,计算乳头矫正高度(术前术后高度差),评估乳头形态。结果 14例患者术后均无乳头血运障碍或感觉障碍,1例患者左侧乳头感觉稍麻木,3个月后恢复正常。其中乳头形态良好者12例,形态一般者3例,形态差者0例。平均矫正高度为0.35 cm。结论采用真皮脂肪瓣联合"┼"字缝线矫正重度乳头内陷,手术效果良好,术后复发率低,是较为理想的乳头内陷矫正方法。Objective To explore the effect of Dermal - fat flaps combined with cross suture support for the correction of severe nip-ple inversion. Methods Fifteen cases with inversed nipple （29 nipples） admitted toAnhui Provincial Hospital between Ju n e ,2016 and A- pril,2017were recruited in the study. Dermal - fat flaps were used to support the basement of nipple, and cross suture under flaps combined with tractor was used to pull nipple for four weeks. Then all cases were followed up for 6 to 12 months, the corrective height （preoperative and postoperative height difference） wascalculated and the nipple morphology was evaluated. Results None of 14 cases appeared nipple blood circulation or sensation disorder after operation, so that the nipple inversion was completely corrected. One case showed mild sensation disorderwith left nipple and it was recovered to normal after three months. Among them, twelve cases had good nipple morphology, three cases had normal morphology and no case had bad shape. The average correction height was 0. 35 cm. Conclusion In the treatment of severe nipple inversion, the technique of the Dermal - fat flaps combined with cross sutureshow perfect effect with less recurrence rate, which is an ideal method for correcting the nipple inversion.
