中医外治法结合微波腔内闭合术治疗下肢静脉曲张伴溃疡  

External Therapy of Traditional Chinese Medicine Combined with Microwave Intracavitary Coagulation in Treating Venous Ulcer of Lower Extremity

摘　　要：目的:评价中医外治法结合微波腔内闭合术治疗下肢静脉曲张伴溃疡的临床疗效。方法:将符合标准的下肢静脉曲张伴溃疡患者66例随机分为治疗组和对照组各33例,均采用微波腔内闭合术,手术前后治疗组采用中医外治法,对照组采用西医常规换药。比较两组溃疡愈合率、溃疡愈合时间、溃疡愈合后复发率、中医证候积分。结果:治疗2个月后,治疗组溃疡愈合率(93.23%)与对照组溃疡愈合率(86.49%)相比有统计学意义(P<0.05)。治疗组临床疗效(痊愈25例,显效8例)优于对照组(痊愈18例,显效11例,有效4例)(P<0.05)。治疗组中医证候疗效亦优于对照组(P<0.05)。治疗3个月后,治疗组平均溃疡愈合时间(33.79±25.28)d短于对照组(48.42±31.63)d(P<0.05)。随访溃疡愈合者6个月,治疗组溃疡复发率(3.03%)低于对照组(20.69%)(P<0.05)。结论:中医外治法结合微波腔内闭合术治疗下肢静脉曲张伴溃疡,疗效显著,溃疡愈合率高,愈合时间短,复发率低。Objective To evaluate the clinical efficacy of the external therapy of TCM combined with microwave intracavitary coagulation in treating venous ulcer of lower extremity.Methods Sixty-six patients with venous ulcer of lower extremity were randomly and equally divided into treatment group and control group.All patients were treated with microwave intracavitary coagulation,and the treatment group received the external therapy of TCM,while the control group received the Western conventional surgical dressing before and after surgery.Then the healing rate of ulcer,the healing time of ulcer,the rate of recurrent ulcer and the TCM syndrome scores were compared between the two groups.Results After two months of treatment,the healing rate of ulcer（93.23%） of the treatment group was significantly higher than that of the control group（86.49%,P〈0.05）.The clinical efficacy of the treatment group（twenty-five cases were cured and 8 cases had a significant effect）was better than that of the control group（eighteen cases were cured and eleven cases had a significant effect and four were effective）.The TCM syndrome efficacy of the treatment group was better than that of the control group.After three months of treatment,the average healing time of ulcer（33.79±25.28）d in the treatment group was shorter than that of the control group[（48.42±31.63）d,P〈0.05].The patients with ulcer healing were followed up for six months,the rate of recurrent ulcer（3.03%） in the treatment group was lower than that of the control group（20.69%,P〈0.05）.Conclusion The external therapy of TCM combined with microwave intracavitary coagulation in treating venous ulcer of lower extremity have a significant effect and a higher healing rate of ulcer and a shorter healing time of ulcer and a lower rate of recurrent ulcer.
