摘  要:目的检测骨科患者感染鲍曼不动杆菌16S rRNA基因分布情况,分析菌株耐药性,为有效的临床治疗提供指导。方法收集骨科患者临床送检标本,分离并鉴定鲍曼不动杆菌,PCR法进行菌株16S rRNA基因的检测,K-B法分析菌株耐药性。结果 48株鲍曼不动杆菌临床分离株对氨基糖苷类抗菌药物阿米卡星、链霉素、庆大霉素、妥布霉素、卡那霉素的耐药率分别为12.50%、56.25%、43.75%、47.92%和27.08%,对链霉素的耐药率最高。对鲍曼不动杆菌的armA基因、rmtA基因、rmtB基因、rmtC基因、rmtD基因以及nmpA基因均进行了检测,仅检出armA基因。armA基因片段大小为590bp,且该基因阳性率为100.00%。结论骨科患者感染鲍曼不动杆菌对常用氨基糖苷类药物产生了一定程度的耐药性,armA的存在可能与菌株耐药性产生存在关系。Objectives To determine the distribution of 16S rRNA genes of Acinetobacter baumannii infecting orthopedie patients in hospital A and to analyze the drug resistance of strains to guide effective clinical treatment of infections. Methods Clinical samples were collected from orthopedic patients and A. baumannii strains were isolated and identified. The 16S rRNA genes of strains were detected using PCR and the drug resistance of strains were analyzed using the K-B method. Results Forty-eight strains of A. baumannii were isolated and their resistance to aminoglyeoside antibiotics was determined. Strains were resistant to amikacin at a rate of 12.500%, to streptomycin at a rate of 56.250%, to gentamicin at a rate of 43.75%, to tobramycin at a rate of 47. 92%, and to kanamycin at a rate of 27.08%. Strains had the greatest resistance to streptomycin. A. baumannii strains were tested for the armA gene, the rmtA gene, the rmtB gene, the rmtC gene, the rmtD gene, and the nmpA gene, but only the armA gene was detected. The armA gene was 590 bp in length, and the gene was detected at a rate of 100.00%. Conclusion A. baumannii infecting orthopaedic patients was highly resistant to aminoglycoside drugs in common use, and its drug resistance may be related to the presence of the armA gene.
