摘  要:目的观察急性大剂量华支睾吸虫感染大鼠模型中的肝脏病理形态学和血清相关指标的改变。方法 12只5~6周龄雌性Sprague-Dawley(SD)大鼠分为实验组和不感染对照组。实验组灌胃200个华支睾吸虫囊蚴/大鼠,采用改良加藤法检查粪便中虫卵。灌胃5周后剖杀,取肝脏,切片,HE染色后观察病理改变。剖杀前采血,分离血清,检测总胆红素(TBIL)、间接胆红素(DBIL)、碱性磷酸酶(ALP)、谷草转氨酶(AST)、谷丙转氨酶(ALT)、白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)含量,用华支睾吸虫IgG抗体试剂盒检测感染特异性。结果实验组大鼠感染囊蚴30d左右粪便中均查到华支睾吸虫虫卵;肉眼见肝脏肿大、淤血,胆管堵塞明显,HE显示胆管扩张、管壁增厚明显,肝细胞肿胀,部分肝小叶结构破坏,胆管上皮细胞排列不规则水肿、坏死;血清TBIL、DBIL、ALP、AST、ALT、IL-6、TNF-α水平较对照组均显著增高(P<0.05),并产生高水平的华支睾吸虫特异性IgG抗体。结论急性大剂量华支睾吸虫感染时肝脏病理形态学和血清指标显著性改变,表明存在严重的肝脏损伤,应予以重视。本研究对探讨人体急性感染华支睾吸虫的致病性具有一定借鉴。To observe changes in serological indices and liver pathology/morphology in rats caused by an a- cute massive infection with Clonorchis sinensis. Methods Fiveto siweek old female SD rats were used in this experi- ment. Rats were orally infected with 200 metacercariae of C. sinensis to establish a model of acute infection (n= 7). Nor- mal rats served as controls (n=-5). The Kato technique was used to detect eggs in stool. HE staining was used to identify pathological changes 5 weeks post-infection. Levels of total bilirubin (TBIL), indirect hilirubin (DBIL), alkaline phos phatase (ALP), aspartate aminotransferase (AST), alanine aminotransferase (ALT), IL-6, and tumor necrosis factor (TNF-a) in serum were measured to evaluate liver function. C. sinensis-specific IgG antibodies were used to determine the specificity of the infection. Results C. sinensis eggs were found in stool 30 days post-infection. Macroscopically, rats in the experimental group had a markedly enlarged liver, marked congestion of blood, and marked blockage of bile ducts. HE staining revealed dilation and thickening of the walls of bile ducts, swollen hepatoeytes, some hepatic lobules with a disrupted structure, intrahepatic cholangiocytes in disarray, edema, and necrosis. The levels of TBIL (76.29±34. 25 tLmol/L), DBIL (35.44±5.68 ptmol/L), ALP (41.43±13.39 U/L), AST (54.31±6.53 U/gprot), ALT (93.55± 14.21 U/gprot), IL-6 (73.26±8.53 pg/ml), and TNF-a (328.61± 17. tZ pg/ml) in serum or plasma from rats in the experimental group were higher than levels of TBIL (14.41±0.51 μmol/L), DBIL (6.38±1.24 μmol/L), ALP (23.10 ±3.03 U/L), AST (11.22±3.38 U/gprot), ALT (12.36±2.56 U/gprot), IL-6 (22.4±4.87 μg/ml), and TNF-a (55.17±10.34 μg/ml) in the control group. Specific IgG antibodies indicated that the OD value was higher in infected rats than in uninfected rats. Conclusion Significant changes in liver pathology/morphology and serological indices were
