摘 要:目的通过分析烧伤创面感染与金黄色葡萄球菌毒力基因的相关性,为临床合理用药提供指导。方法收集河北医科大学第一医院烧伤创面感染患者送检标本分离金黄色葡萄球菌。PCR扩增毒力基因,并采用统计学分析烧伤创面感染与金黄色葡萄球菌毒力基因的相关性。结果分离出149株金黄色葡萄球菌,其中分离自痰液95株,占63.76%;脓液21株,占14.09%;血液16株,占10.74%;分泌物11株,占7.38%;其他标本6株,占4.03%。PCR扩增检测细菌毒力基因,sasX、psm-mec、pvl毒力基因大小分别为615、210和156bp,检出率分别为8.05%、2.82%和54.36%。分析烧伤创面感染与金黄色葡萄球菌毒力基因sasX、psm-mec和pvl的相关性,OR值分别为0.8147(95%CI为0.2464~2.6935)、2.2548(95%CI为1.0287~4.9422)和0.3042(95%CI为0.1551~0.5966)。结论金黄色葡萄球菌毒力基因psm-mec与烧伤患者创面感染程度有关,与sasX和pvl无关。Objective To analyze the correlation between infection of burn wounds and virulence genes of Staphylococ- cus aureus to guide rational drug use in clinical practice. Methods Samples from patients with infected burn wounds at the First Hospital of Hebei Medical University were collected and S. aureus was isolated. Virulence genes were amplified using PCR, and the correlation between infection of burn wounds and virulence genes of S. aureus was statistically ana lyzed. Results One hundred and forty-nine strains of S. aureus were isolated, including 95 strains (63. 76%) from sputum samples, 21 strains (14. 09%) from pus samples, 16 strains (10.74%) from blood samples, 11 strains (7.385) from samples of secretions, and 6 strains (4.03 %) from other samples. Virulence genes were amplified with PCR. sasX was 615 bp in length, psm-mec was 210 bp in length, and pvl was 156 bp in length, sasX was detected in 8.05%if0 of the strains, psm-mec w was detected in 2.82%, and pvl was detected in 54.36%if0. The correlation between infection of burn wounds and virulence genes of S. aureus was analyzed, sasX had an OR of 0. 8147 (95%CI: 0. 2464-2. 6935), psm-mec had an OR of 2.2548 (95%CI: 1.0287-4. 9422), pvl had an OR of 0. 3042 (95CI.. 0.1551-0.5966). Conclusion The virulence gene psm-mec of S. aureus was related to the severity of infection in patients with infected burn wounds, but the genes sasX and pvl were not related to the severity of infection.
