养血清脑颗粒配合椎针治疗68例偏头痛(基底动脉型)临床观察

摘　　要：目的观察养血清脑颗粒配合椎针治疗偏头痛(基底动脉型)的临床疗效。方法偏头痛(基底动脉型)患者136例随机分为对照组与观察组,对照组采用养血清脑颗粒治疗,观察组以养血清脑颗粒配合椎针疗法治疗,均4周为1疗程。运用经颅多普勒超声(TCD)观察治疗前后各组椎动脉(VA)-基底动脉(VBA)收缩峰血流速度(Vp)、舒张末血流速度(Vd)及其平均血流速度(Vm)的变化。结果观察组改善椎-基底动脉收缩峰及平均血流速度方面疗效显著优于对照组(P<0.05);观察组头痛程度NRS评分显著低于对照组(P<0.05);两组总有效率分别为91.18%、76.47%,观察组显著高于对照组(P<0.01)。结论养血清脑颗粒配合椎针治疗可以显著改善偏头痛(基底动脉型)患者椎-基底动脉缺血状态,临床疗效显著不易复发,值得临床推广应用。Objective To observe the efficacy of treated by Yangxue Qingnao granule combined with vertebral needle on basilar artery type of migraine. Methods 68 cases of patients with basilar artery type of migraine were randomly divided into control group and observation group randomly. Control group was treated by Yangxue Qingnao granule, observation group was treated with Yangxue Qingnao granule combined with vertebral needle, the courses were 4 weeks. Changes of the contraction peak, the end - diastolic and average blood flow velocity of VBA before and after treatment in both groups were observed by transcranial Doppler （TCD）. Results The contraction peak, the end - diastolic and average blood flow velocity of VBA in observation group was supior to control group （P 〈 0.05 ）. The headache severity score of NRS of observation group was significantly lower than that of control group （ P 〈 0.05 ）. The total efficacy rates were 91.18% in observation group and 76.47% in control group, observation group was significantly higher than that of control group（ P 〈 0.01 ）. Conclusion Yangnue Qingnao granule combined with vertebral needle therapy can significantly improve the vertebrobasilar artery ischemia of patients with basilar artery type of migraine with satifactory clinical effect and low recurrence rate, which is worthy of clinical application.
