摘 要:目的评价恒温扩增芯片法在下呼吸道感染病原体检测中的效果。方法收集合格痰标本94例,通过恒温扩增芯片法和细菌培养法检测病原体,痰涂片抗酸染色、RNA恒温扩增,静脉血γ干扰素释放试验检测结核分枝杆菌。结果 42例标本通过恒温扩增芯片法检测到病原体,阳性率44.7%。主要病原体为流感嗜血杆菌(13株)、肺炎链球菌(10株)、铜绿假单胞菌(9株)、结核分枝杆菌(8株)、金黄色葡萄球菌(6株)、肺炎克雷伯菌(7株)、大肠埃希菌(4株)、嗜麦芽窄食单胞菌(4株)、鲍曼不动杆菌(1株)、衣原体(1株),未检测到肺炎支原体、嗜肺军团菌。结论恒温扩增芯片法操作简便、快速,可同时检测12种病原体并可筛选耐甲氧西林葡萄球菌的mecA基因。其对流感嗜血杆菌、肺炎链球菌检出率显著高于传统细菌培养法,对结核分枝杆菌的检测显著优于γ干扰素释放试验和涂片抗酸染色。该技术有利于下呼吸道感染的快速诊断和及时地针对性治疗。Objective To evaluate the effect of isothermal amplification chip method in lower respiratory tract pathogens detection.Methods A total of 94 qualified sputum samples were collected.The pathogens were detected by using the isothermal amplification chip method and bacterial culture method,Mycobacterium tuberculosis was tested by using the Ziehl-Neelsen staining,RNA isothermal amplification and interferon gamma releasing assay(IGRA).Results Pathogens were detected by using the isothermal amplification chip method in 42 specimens with the positive rate of 44.7%,which were mainly Haemophilus influenzae(13strains),Streptococcus pneumoniae(10strains),Pseudomonas aeruginosa(9strains),Mycobacterium tuberculosis(8strains),Staphylococcus aureus(6strains),Klebsiella pneumoniae(7strains),Escherichia coli(4strains),Stenotrophomonas maltophilia(4strains),Acinetobacter baumannii(1strains),Chlamydia pneumoniae(1strain).Mycoplasma pneumoniae,legionella pneumophila were not detected.Conclusion The isothermal amplification chip method is easy to operate and rapid,which can detect 12 kinds of pathogens,screen methicillin-resistant mecA gene in staphylococcus.Its detection rates for Haemophilus influenzae and Streptococcus pneumoniae are significantly higher than those of traditional bacterial culture.The detection of Mycobacterium tuberculosis is obviously superior to IGRA and ZiehlNeelsen staining.This technique is conducive to the rapid diagnosis of low respiratory tract infection and timely targeted treatment.
